Position Summary

Harvest Healthcare Services is seeking a Certified Occupational Therapist Assistant (COTA) to provide high-quality occupational therapy services under the direction of a licensed Occupational Therapist. This position plays a vital role in helping patients regain functional independence through individualized treatment plans designed to improve daily living skills, mobility, strength, safety, and overall quality of life. Responsibilities:

  • Provide occupational therapy treatments in accordance with physician orders, state practice guidelines, and established plans of care. 
  • Assist the Occupational Therapist with resident evaluations, treatment planning, and goal development. 
  • Monitor patient progress and communicate changes or recommendations to the Occupational Therapist. 
  • Document treatments accurately and maintain timely therapy and billing documentation. 
  • Educate patients, families, and caregivers on therapeutic techniques, adaptive equipment, home programs, and safety recommendations. 
  • Coordinate therapy schedules with the interdisciplinary care team. 
  • Participate in discharge planning and recommend appropriate follow-up services. 
  • Supervise rehabilitation aides, students, or volunteers as permitted by state regulations. 
  • Participate in Quality Assurance activities and departmental meetings. 
  • Maintain compliance with HIPAA, infection prevention standards, OSHA regulations, and company policies. 
  • Promote a safe, respectful, and patient-centered rehabilitation environment. 
  • Perform other duties as assigned. 

Qualifications

  • Associate’s degree from an accredited Occupational Therapy Assistant program. 
  • Current Certified Occupational Therapist Assistant (COTA) license in the state where services are provided. 
  • Current CPR certification (preferred or required based on facility). 
  • Strong communication, organization, and documentation skills. 
  • Ability to work collaboratively with therapists, nursing staff, physicians, residents, and families. 
  • Experience in skilled nursing, rehabilitation, or long-term care is preferred but not required. 
  • Commitment to continuing education and professional growth.

Physical Requirements

This position requires the ability to:

  • Stand, walk, bend, kneel, squat, and reach throughout the workday. 
  • Safely assist with patient transfers, mobility, and therapeutic activities. 
  • Lift up to 50 pounds with or without reasonable accommodation. 
  • Operate therapy equipment and standard office equipment. 
  • Maintain predictable attendance and travel between assigned facilities as needed.
