What You’ll Be Doing
The Coordinator is responsible for independently planning, directing, coordinating, and overseeing the administrative and supervisory functions necessary to ensure effective service delivery, regulatory compliance, personnel management, and operational continuity for assigned individuals and support staff. The Coordinator manages assigned caseload operations, provides leadership to Personal Care Attendants (PCAs), and exercises independent judgment and discretion in matters affecting agency operations, staffing, client services, and compliance with agency policies and Tri-Counties Regional Center (TCRC) requirements.
This position serves as the primary administrative liaison between New Heights Ventures, TCRC, medical providers, families, landlords, community agencies, and other service providers to ensure the delivery of high-quality, person-centered services. The Coordinator is responsible for interpreting and implementing agency policies, monitoring program effectiveness, resolving operational and personnel issues, ensuring regulatory compliance, and making effective recommendations regarding staffing and personnel decisions.
The Coordinator's primary duty consists of performing exempt administrative and supervisory functions requiring the consistent exercise of discretion and independent judgment with respect to matters of significance affecting agency operations, personnel management, regulatory compliance, and service delivery. The Coordinator reports directly to members of the Executive Team.
Essential Duties
- Independently manage the day-to-day administrative operations of assigned caseloads while ensuring continuity of services and compliance with agency policies and applicable regulatory requirements.
- Direct, assign, supervise, and evaluate the work of assigned Personal Care Attendants (PCAs), ensuring staff performance meets agency expectations and quality standards.
- Conduct staff onboarding, provide ongoing training, coaching, mentoring, and performance evaluations, and address employee performance concerns through corrective coaching and documented performance management.
- Make effective recommendations regarding hiring, promotion, discipline, retention, termination, scheduling, and staffing assignments, with recommendations given substantial consideration by agency leadership.
- Exercise independent judgment in resolving staffing challenges, operational issues, client concerns, and service delivery matters with minimal supervision.
- Develop, coordinate, modify, and monitor implementation of individualized service plans to ensure services remain consistent with TCRC authorizations, person-centered planning principles, agency policies, and client needs.
- Monitor service quality, identify barriers to successful service delivery, implement corrective action when appropriate, and recommend operational improvements that enhance program effectiveness.
- Interpret and apply agency policies, TCRC requirements, and applicable state and federal regulations to ensure ongoing compliance in service delivery and documentation practices.
- Establish, review, and monitor administrative systems to ensure all required documentation, reports, medication oversight records, incident reports, and client files are accurate, complete, and submitted within required timelines.
- Review documentation completed by assigned staff to ensure compliance with agency standards, regulatory requirements, and quality assurance expectations.
- Collect, analyze, and communicate relevant client information to TCRC, agency leadership, medical providers, and other authorized stakeholders to facilitate informed decision-making and continuity of care.
- Serve as the primary liaison with TCRC Service Coordinators, family members, physicians, behavioral health providers, landlords, community agencies, and other service providers to coordinate services and resolve issues impacting client care.
- Coordinate and oversee services supporting independent living, including housing stability, budgeting support, medical care coordination, medication oversight, transportation planning, and community integration while ensuring services remain person-centered and compliant with authorized plans.
- Investigate service concerns, incidents, and operational issues; implement appropriate corrective actions; and recommend policy or procedural improvements to agency leadership.
- Participate in quality assurance activities, compliance reviews, audits, and program evaluations to identify opportunities for continuous improvement.
- Respond to urgent situations requiring independent decision-making and participate in the agency's emergency on-call rotation to ensure uninterrupted service delivery.
- Attend interdisciplinary meetings, annual planning meetings, quarterly reviews, and other required meetings representing the agency and advocating for appropriate client services.
- Fulfill mandated reporting responsibilities by promptly reporting suspected abuse, neglect, exploitation, or financial abuse to Adult Protective Services (APS) or other appropriate authorities in accordance with California law and agency policy.
- Support agency-wide initiatives and perform other administrative and supervisory responsibilities as assigned to promote operational excellence and quality service delivery.

Qualifying Specifications:
- Minimum of two (2) years of experience working with adults with developmental disabilities or a closely related population.
- Previous supervisory or leadership experience with responsibility for directing staff performance preferred.
- Demonstrated ability to independently exercise discretion and sound judgment in managing personnel, operations, and client service issues.
- Strong knowledge of person-centered planning, independent living services, and regional center systems preferred.
- Excellent organizational, leadership, communication, conflict resolution, and problem-solving skills.
- Ability to establish and maintain effective professional relationships with employees, clients, families, funding agencies, medical providers, and community partners.
- Ability to manage multiple priorities, adapt to changing operational needs, and respond effectively during emergencies.
- Strong written communication, documentation, and record management skills.
- Proficiency with computer systems and electronic documentation platforms.
- Current First Aid and CPR certification, or ability to obtain certification upon hire (agency reimbursement available).
- Valid California driver's license, reliable transportation, and a driving record meeting company requirements.

Physical Requirements
The employee must be able to perform the essential functions of the position with or without reasonable accommodation. Duties require regular travel between client worksites, prolonged periods of standing or walking, occasional lifting or moving of household items or supplies, and the ability to respond to emergency situations within assigned service areas.
