Position Summary

The Physical Therapist provides skilled, patient-centered care in the home setting and uses clinical expertise to support mobility, safety, independence, and quality of life. This role is ideal for a licensed professional who values meaningful patient relationships, clinical autonomy, and collaboration with an interdisciplinary care team.


Schedule

  • Full-time position
  • Monday-Friday
  • Weekday schedule

Requirements
  • Current Physical Therapist license in the State of Arizona
  • Graduate of an accredited Physical Therapy program
  • Current CPR certification
  • Valid driver’s license, reliable transportation, and automobile insurance that meet agency requirements
  • Strong clinical judgment, organization, and communication skills
  • Ability to work independently while collaborating effectively with an interdisciplinary team
  • Home health experience is preferred
  • Commitment to compassionate, patient-centered care
