Position Summary
The Physical Therapist provides skilled, patient-centered care in the home setting and uses clinical expertise to support mobility, safety, independence, and quality of life. This role is ideal for a licensed professional who values meaningful patient relationships, clinical autonomy, and collaboration with an interdisciplinary care team.
Schedule
- Full-time position
- Monday-Friday
- Weekday schedule

- Building trusted relationships with patients, families, and caregivers

- Enjoy a consistent weekday schedule, Monday through Friday
- Have a desire to work with an organization committed to compassionate, high-quality care
- Current Physical Therapist license in the State of Arizona
- Graduate of an accredited Physical Therapy program
- Current CPR certification
- Valid driver’s license, reliable transportation, and automobile insurance that meet agency requirements
- Strong clinical judgment, organization, and communication skills
- Ability to work independently while collaborating effectively with an interdisciplinary team
- Home health experience is preferred
- Commitment to compassionate, patient-centered care
