The Case Manager provides comprehensive case management services to youth and families participating in violence prevention, youth development, and survivor support programs. The Case Manager works collaboratively with schools, community partners, caregivers, and service providers to assess participant needs, develop individualized service plans, coordinate referrals, monitor participant progress, and ensure successful engagement in program services.
The Case Manager serves as a key advocate for participants by connecting them to educational, behavioral health, social service, and community resources that promote safety, stability, well-being, and positive youth outcomes.
- Maintain resource directories and referral partnerships.
- Monitor participant progress and update service plans as needed.
- Assist participants in accessing educational, employment, healthcare, housing, mental health, legal, and social service resources.
- Provide crisis intervention and support during critical incidents.
- Maintain regular contact with youth and families through individual meetings, phone calls, home visits, and virtual communication as appropriate.
- Develop individualized service plans based on participant strengths, goals, and identified needs.
- Conduct participant intake, needs assessments, and risk screenings.
- Coordinate services with schools, community organizations, healthcare providers, behavioral health professionals, and other stakeholders.
- Engage parents, caregivers, and support systems in participant goal planning and service delivery.
- Conduct outreach activities to promote program participation and community awareness.
- Facilitate or support caregiver workshops, support groups, and community events.
- Advocate on behalf of participants and families to reduce barriers to services.• Participate in staff meetings, planning sessions, and professional development activities.
- Support workshop facilitation and youth programming activities.
- Assist with participant recruitment, retention, and engagement efforts.
- Contribute to continuous quality improvement initiatives and program evaluation activities
- Collaborate with program staff to ensure participants receive comprehensive and coordinated services.
Education
• Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Criminal Justice, Education, or a related field.
Experience
• Minimum of two years of experience providing case management, youth services, social services, or community-based support.
• Knowledge of trauma-informed care, strengths-based practices, and youth development principles.
• Strong interpersonal, organizational, and problem-solving skills.
• Ability to maintain confidentiality and professional boundaries.
• Proficiency with Microsoft Office and electronic case management systems.
• Experience working with youth exposed to violence, trauma, behavioral health challenges, or justice system involvement.
CONDITIONS OF EMPLOYMENT
- Ability to work varying hours Monday - Friday; schedule may include some evenings and weekends.
- Must have a valid driver’s license and automobile insurance required for work-related travel.
PHYSICAL REQUIREMENTS
- Must be able to remain stationary 50% of the time.
- Must be able to frequently move inside the office to access file cabinets, office machinery, etc.
- Must be able to transport oneself to and from home visits.
- Occasionally ascends/descends stairs.
- Constantly moves to assist during program activities.
- At times, it may be required to lift up to 25 lbs. for various needs.
- Frequently moves in outdoor weather conditions.
