Summary:


InterCommunity HealthCare is currently seeking a Community Health Navigator to join our team. The Community Health Navigator plays a vital role in connecting clients to essential health services. This position supports individuals by coordinating appointments, providing transportation, participating in community outreach, and promoting both physical and mental well-being. The ideal candidate is compassionate, organized, and knowledgeable in basic medical and behavioral health concepts.

Essential Duties & Responsibilities:

Client Support & Navigation

  • Schedule and coordinate medical, behavioral health, and social service appointments.
  • Provide safe and timely transportation to and from appointments and community events.
  • Assist clients in understanding care plans and navigating healthcare systems.

Community Engagement

  • Represent the organization at community events, health fairs, and outreach activities.
  • Build relationships with local partners and service providers to enhance client support.

Health Education & Advocacy

  • Educate clients on the importance of preventive care, chronic disease management, and mental health awareness.
  • Advocate for client needs within the healthcare system and community services.

Documentation & Communication

  • Maintain accurate records of client interactions, appointments, and services provided.
  • Communicate effectively with clinical teams to ensure continuity of care.
  • Collaborating with healthcare professionals to ensure efficient patient care

Additional Responsibilities:

  • Maintaining cleanliness and organization in patient care areas
  • Maintain accurate and up-to-date documentation of patient assessments, care plans, progress notes, and services rendered.
  • Ensure compliance with regulatory requirements and healthcare policies and procedures.
  • Adhere to all federal and state regulations and guidelines.
  • Always maintain patient confidentiality.
  • Perform other duties as assigned

Schedule:


Monday - Friday, 8:00 AM - 4:30 PM

Requirements

Qualifications: 

  • High school diploma or equivalent required; associate degree or healthcare certification preferred.
  • Valid driver’s license and reliable transportation required.
  • Strong interpersonal, organizational, and communication skills.
  • Empathy and cultural sensitivity
  • Problem-solving and resourcefulness
  • Team collaboration and independence

Education &/OR Experience:

  • Experience in community health, social services, or a healthcare setting.
  • Understanding physical and mental health conditions.
  • Commitment to health equity and community wellness
  • At least 2-3 years of experience working in a medical setting.
  • Bilingual English to Spanish preferred 
  • All Agency Staff Employees MUST show proof of CPR Certification within 60 days of official start date. 



Salary Description
Wage Range: $21.25 - $25.00 Hourly
