Physical Therapist Assistant (32 hrs/wk)

Requirements

 Minimum Qualifications

· Graduate of a 2-year college level program, approved by the American Physical Therapy Association.

· Must be licensed in Wisconsin.

· Home health and/or hospice experience preferred.

· Current Wisconsin driver's license.

· Excellent communication skills.

· Evidence of attendance of hospice training class or willingness to attend.

· Believes in and has a working knowledge of the hospice philosophy of care.

· Basic computer skills

As a PTA at Adoray, you will 

Follow the Physical Therapist's plan of treatment

Notify appropriate team members of any deviations from the plan of treatment and reporting observations

Provide teaching to patients and/or caregivers

Report changes in patient's condition

Complete appropriate clinical documentation

Maintains own work schedule and provides updated schedule

Maintains current CPR (BLS) certification

Accepts special assignments based on interest and knowledge base

Demonstrates efficient use of time in visiting, travel time, coordination with others, and documentation.

Always demonstrates respectful and professional communication with patients and coworkers
