Bishop Gadsden is seeking a compassionate, energetic, and relationship-focused Care Navigator Companion to provide non-medical support, transportation, companionship, and personalized engagement for our residents.

As a Care Navigator Companion, you'll help residents maintain independence, stay socially connected, and enjoy a higher quality of life. You'll serve as a trusted support system while building rewarding relationships with seniors and their families.

  • Accompany residents to appointments, dining, errands, community outings, and social activities
  • Coordinate and provide transportation as needed
  • Create personalized engagement opportunities that enhance quality of life
  • Communicate resident updates to the Care Navigation team and maintain accurate documentation
  • Assist with appointment scheduling, move management, and other resident support services
  • Collaborate with residents, families, and team members to deliver an exceptional experience
Requirements

What We Need In A Candidate:

  • Must be at least 21 years old
  • Associate's Degree in Healthcare, or a related field preferred
  • High School Diploma or GED required
  • Valid South Carolina Driver's License with a clean driving record (no infractions within the last 3 years)
  • Minimum of 2 years of experience in case management, senior care, elder care, or a related field
