Job Description:

The Cypress Center is looking to fill an opening for a second-shift RN Inpatient Admission Specialist. Our candidate will be hired for a unique schedule of 4 pm to midnight (5 evenings per week, with some flexibility on working one weekend evening if preferred). The position will allow for our designated receiving facility to better process referrals from CTC and emergency departments statewide after hours, as well as work directly with our admitting providers to assess the medical necessity for a non-hospital-based inpatient level of care.


Key Responsibilities:

  • Receive and review all referrals that are faxed to Cypress Center Unit (CCU)
  • Complete prescreening information, and arrange referrals for presentation to the doctor
  • Reach out to referral sources to confirm receipt of faxes, communicate hourly on what is needed for missing information, and ensure that the person is still in need of a bed
  • Communicate directly with Care Traffic Control team at the state hospital, as well as with emergency department nurses/clinicians throughout the day on our bed status and availability
  • Present all referrals to our admitting doctors and act as an advocate for referrals to our unit, including those with dual disorders in need of MAT intervention
  • Complete all nurse-to-nurse communications with sending facilities
  • Complete prior authorizations and concurrent/discharge reviews for insurance when needed or when time allows.
  • Complete eligibilities for patients who have been admitted to CCU over 10 days.
  • Work with referral sources on completing medication reconciliations and communicating pharmacy needs for new admissions with CCU nurses.
  • Reach out to all state referral sources hourly when we have open beds to give real-time status on unit acuity, open referrals, and information still needed.
  • Act as a liaison connecting our daytime and evening shifts so that information regarding new admissions/referral status is passed on verbally from those who will be completing daytime admissions.
  • Sit in on phone huddles with local hospitals, CTC, and stakeholders to offer information on current admissions statuses.
  • Attend Rapid response/ES shift report huddles to develop a stronger link of communication regarding daily bed availability at the Cypress Center and to identify potential barriers to receiving critical information for referrals in our queue.
  • Other duties as required.
Requirements

Knowledge, Skills, and Abilities:

  • Knowledge of levels of care within the mental health system in NH
  • EXCELLENT customer service skills
  • Ability to initiate tasks without delay and advocate for patients with a wide variety of mental health crises (which may include substance misuse)
  • Solid knowledge of emergency department review of systems and ability to read lab work quickly and efficiently to identify potential areas of concern in the admission process
  • Must be able to think critically yet positively…and remain free of stigma in all communications.
  • Must be able to work independently, without distraction, as well as part of a multidisciplinary team.
  • Bilingual candidates are encouraged to apply.

Qualifications:

RN with some experience in a psychiatric acute care environment. May consider an LPN with the right experience.


Experience:

Our ideal candidate will have at least 1 year working in a psychiatric setting, particularly in an acute care environment.
