Silver Maples of Chelsea is hiring a full time day/evening split as a Supervisor in our Meadow's Assisted Living Department. This position qualifies for a comprehensive benefit package including Health, Dental, Vision, Life, Short & Long Term Disability and 401K with immediate employer match!


Job Summary: The Meadows Assisted Living Supervisor is responsible for managing the resident’s service plan and the staff who are delivering care. The Supervisor is responsible to the Department Director and/or Clinical Coordinator. Must be able to assess resident needs and be able to plan and implement resident services; which are documented and communicated via the “Service Plan” as mandated by the HFA regulations. Must effectively interact with residents, families and staff to ensure that quality care and services are provided. Ensures compliance of State Rules and Regulations of Homes for the Aged licensing.


Major Duties:

  • Provide leadership, coaching and mentoring to Resident Care staff. Provide direction and supervision of tasks.
  • Participate in the admission of new residents and in the coordination of medical care for residents.
  • Responsible for non-urgent and emergent response to all IL residents.
  • Ensure compliance of State Rules and Regulations of Homes for the Aged licensing.
  • Assist residents, family members, and other clients with concern and empathy, respect their confidentiality and privacy and communicate with them in a courteous and respectful manner.
  • Oversee and ensure that physician orders are followed. Must be able to read, interpret, and follow up on abnormal lab work in conjunction with the physician.



Requirements

Minimum Qualifications:


  • Three years direct care in geriatric setting, including experience with memory care (i.e. nursing homes, assisted living, home care, hospice, etc.).
  • Direct supervisory experience, minimum one year.
  • Must have medical knowledge and ability to speak knowledgably with physicians, social workers, EMT’s, discharge planners, families, emergency room personnel, and other persons involved with resident care. Must be able to accurately read, interpret, and process physician orders.
  • Must demonstrate excellent written and verbal communication skills
  • Computer skills required. Experience with Microsoft Office, Word and Excel preferred with the ability to learn our Medical Charting system and EMAR functions.
  • Registered LPN preferred, or other formal medical education & training.
