Elders' Program Coordinator

The Elder Care Program Coordinator supports Tribal Elders through frequent home visits, welfare checks, needs assessments, and connection to medical, social, community, and Tribal resources. The position builds trusting relationships with Elders, identifies changes or unmet needs, assesses safety and daily living needs, coordinates services and care plans, and works with Elders, families, caregivers, healthcare providers, and service providers to support the Elder’s well-being and independence. The position also maintains accurate documentation and case records and follows through to ensure identified needs, referrals, and services are addressed.


Essential Duties:

Level I -   

  • Conduct home visits and welfare checks with Tribal Elders to assess and monitor their physical, social, emotional, cultural, and environmental well-being; identify changes in condition, safety concerns, isolation, unmet needs, or barriers to independent living; and determine appropriate follow-up, resources, referrals, or support services.
  • Evaluate the effectiveness of the care plans and recommend adjustments as needed.
  • Maintain outreach and provide support services to Tribal Elders based on individual needs.
  • Conduct Activity of Daily Living Assessments to Determine eligibility for program services.
  • Connect Tribal Elders with essential community resources, providing hands-on, in-person assistance as needed.
  • Prepare and submit payment requests and Elder In-Home Care Timesheets for supervisor review and approval.
  • Assist Tribal Elders with navigating resources such as Washington Connections, Apple Health and DSHS Long-Term Services and Supports; while collaborating with medical professionals and other service providers to support coordinated care.
  • Coordinate transportation for health-related appointments as needed.
  • Maintain transportation logs, reports, and related program documentation.
  • Prepare purchase requests for program services, supplies, and activities for program manager review and approval.
  • Participate in Elder program activities and events in a respectful and supportive manner.
  • Maintain accurate client records, case notes, and other required program documentation.
  • Participate in department meetings and trainings as appropriate.
  • Assist Tribal Elders’ physical and mental wellness, needs, preferences, and abilities to develop individualized care plans.
  • Coordinate services and care plans for Tribal Elders by working with Elders, families, caregivers, healthcare providers, Tribal programs, and state or community resources; assist with identifying appropriate services and resources; and follow up to ensure identified needs, referrals, and care coordination plans are addressed.
  • Document Trial Elders’ progress, including charting referrals, home visits, and other notable interactions
  • Evaluating Tribal Elders’ progress periodically and make adjustments to care plans or services as needed to improve outcomes.


Level II - 

Performs all duties of Level I, plus: 

  • Assist with program policy development as needed.
  • Utilize Tribal Assister certification to assist Elders with enrollment in Apple Health and other eligible assistance programs.

Additional Duties:

  • Assist in identifying Tribal Elder needs to ensure optimal support and care.
  • Participate in program staff meetings, provider meetings, and workgroups as needed.
  • Attend required trainings and continuing education necessary to effectively perform job duties.
  • Assist with Title VI Elder Needs Assessment Survey and other surveys for information.
  • The Tribe values its commitment to community service and the well-being of its members. In recognition of this, each employee may be expected to perform duties from time to time which may not be related to their regular responsibilities.

Requirements
  • Proficient in computer skills including Microsoft Office applications.
  • Proficient in navigating DSHS resources and applications.
  • Knowledge of Medicare and Social Security Administration resources and benefit programs.
  • Case management experience
  • Strong organizational skills with attention to detail and accuracy.
  • Ability to prioritize workload and meet strict deadlines.
  • Excellent customer service skills and written and verbal communication.
  • Ability to communicate effectively with diverse populations in a courteous, clear, and helpful manner. Knowledge of Native cultures, history, and traditions preferred.
  • Knowledge of age-related health and social conditions affecting seniors preferred. Ability to maintain confidentiality and use tact and discretion.
  • Demonstrate professionalism, strong communication skills, and sensitivity to the needs of Tribal Elders and their families.
  • Demonstrates patience, empathy, and sensitivity when working with Tribal Elders, families, and caregivers; recognizes individual needs, preferences, and concerns.
  • Provides respectful, service-oriented support that promotes the dignity, independence, well-being, and quality of life of Tribal Elders.
  • Ability to observe and identify changes in an Elder’s physical, emotional, social, or environmental condition, including needs or concerns that may not be apparent through telephone communication.

Education & Experience Requirements:

Education Required:  

Level I -   

  • Associate degree in human services, social work, healthcare administration, public health, nursing, or a closely related field preferred; or High School Diploma/GED with directly related experience in elder care, healthcare support, or human services is required. HCA (Home Care Aide) or CNA (Certified Nursing Assistant) certification preferred

Level II – 

  • Bachelor’s degree in social work, public health, human services, or related field; or an equivalent combination of education and experience that demonstrates the ability to perform the essential functions of the position may substitute for degree requirement.

Experience Required:

Level I - 

  • Minimum of two (2) years of experience working with senior citizens is required.
  • Minimum of one (1) year of experience with case management is required
  • Minimum of one (1) year of experience working in food preparation is preferred.
  • Minimum of two (2) years of experience working in Native Community with Elders or Native families is preferred.

Level II – 

  • Minimum of three (3) years of experience working with senior citizens is required.
  • Minimum of two (2) years of experience working in food preparation is required.
  • Minimum of two (2) years of experience supporting aging-related health conditions or in geriatric social work.
  • Minimum of three (3) years of experience working in a Native Community with Elders or Native families is required.


Certifications/Licenses:

Level I - 

  • Must have and maintain a valid Washington State driver’s license.
  • Current CPR/AED Certification.
  • Must obtain a valid Food Handlers Certification within 90 days of employment. 

Level II – 

Requires the certifications/license requirements of level I; plus:

  • Must have and maintain a Certified Tribal Assister certification.


Other Requirements:

  • Must be able to pass a thorough pre-employment background investigation. 
  • Must agree to be subject to the Tribe's Controlled Substance and Alcohol Testing Policy to include pre-employment screening.
  • Must be able to successfully pass the Tribe’s Defensive Drivers Class.

Work Environment:

The position requires frequent travel and home visits and may involve working in a variety of residential environments. The position also includes an office environment in a controlled atmosphere building. The administrative and documentation portion of the position may be eligible for hybrid work arrangements in accordance with the Tribe’s Telecommuting Policy.

The employee may occasionally need to provide reasonable physical assistance to Elders with mobility or daily living activities within the scope of the position, such as assisting with safe movement through the home or handling light equipment or supplies. The position is not intended to require lifting or transferring Elders.

The Snoqualmie Indian Tribe follows the Tribal Hiring Preference as outlined in tribal policies, including the Tribal Employment Rights Ordinance (TERO). TERO sets forth certain hiring preferences for qualified individuals who are enrolled in a federally recognized Indian tribe, as well as, in some cases, preference for certain family members of those individuals.


Additional Information:

  • This is a full-time hourly position and employment is at-will.
  • Compensation will be determined based on experience and starts at $27.21 per hour.

Salary Description
starts at $27.21 per hour
