SUMMARY: Provide individual and/or family mental health assessment and treatment for children, adolescents, and their families in the school-based health center. May provide services to child and adult clients as requested at main sites.
SPECIFIC DUTIES AND RESPONSIBILITES: All duties are subject to accommodation in accordance with the Americans with Disabilities Act (ADA).
- Provides psychosocial assessment, treatment planning, therapy, and appropriate referrals to youth ages 5 through 18 (or upon completion of high school).
- Facilitates group sessions (e.g. CBITS/Bounce Back) as assigned.
- Refers clients for psychiatric care, Southwest services, and outside providers/services as necessary.
- Consults with supervisor, psychiatric providers, department members, and medical staff regarding client care.
- Participates in behavioral health child rounds, individual supervision, and department/agency meetings.
- Provides advocacy for clients within the school system (e.g. 504 and IEP meetings as requested by the client’s guardian) and other systems such as DCF.
- Maintains client records in the EHR in accordance with SWCHC timeframes/policies and provides other documentation as requested.
- Performs other duties as assigned.
Qualifications:
- Excellent organizational and communication skills.
- Computer literate including Microsoft Office; ability to document in an EHR.
- Ability to work well with culturally diverse populations.
- CPR/BLS Certified
Education and/or Experience:
- Graduate degree in Social Work, Counseling or Human Services.
- CT License required. LCSW, LPC, LMFT preferred.
- Experience in providing services to medically underserved and culturally diverse populations.
- Ability to utilize Electronic Health Record (EHR)
