Service Coordinator
Description

 Position Summary


The Service Coordinator is responsible for coordinating and managing personal care services for clients to ensure the delivery of high-quality, compassionate, and compliant care. This role serves as the primary liaison between clients, caregivers, family members, and agency staff, ensuring client needs are assessed, schedules are maintained, and services are delivered effectively.


Key Responsibilities

  • Coordinate and schedule personal care services for clients based on care plans and individual needs.
  • Match caregivers with clients according to skills, availability, and client preferences.
  • Maintain and update client records, service plans, and scheduling systems.
  • Conduct client intake processes and assist with assessments and service authorization requirements.
  • Monitor service delivery to ensure client satisfaction and compliance with agency policies and regulations.
  • Communicate regularly with clients, families, caregivers, healthcare providers, and referral sources.
  • Address and resolve scheduling conflicts, service concerns, and client complaints in a timely and professional manner.
  • Track caregiver attendance, availability, and performance issues.
  • Assist with caregiver orientation, training coordination, and ongoing support.
  • Ensure documentation is complete, accurate, and maintained according to state and agency requirements.
  • Participate in quality assurance activities and client follow-up visits or calls.
  • Support billing, payroll verification, and reporting functions as needed.
  • Maintain confidentiality of client and employee information in compliance with HIPAA and other applicable regulations.
  • Conduct home visits.

  • Ability to maintain confidentiality and professionalism at all times.

Physical Requirements

  • Ability to sit, stand, and use a computer for extended periods.
  • Travel to clients' homes, community locations, or agency meetings may be required.

Working Conditions

  • Office-based environment with frequent phone and electronic communication.
  • May require occasional evening or weekend availability to address urgent client or staffing needs.

Requirements
  • Must pass a background check
  • High school diploma or equivalent 
  • 1-2 years experience in home care, healthcare, scheduling, customer service.  Preferred but not required. 
  • Experience working with elderly, disabled, or vulnerable populations. Preferred but not required. 



Salary Description
$18-$20/Hr
