VNA Hospice NWI is seeking:

Hospice Aides (CNA) or (HHA)

Job Type: Full-Time
Schedule: Monday-Friday with rotating on-call responsibilities
Location: Home Hospice Visits

  • Starting pay $18.50/hour

Minimum On-Call Rotation

Coverage area includes Porter, Lake, Jasper, Starke, Newton and LaPorte. Mileage paid at current IRS rate, if a fleet car is not available.

Must be willing to go into patients' homes and travel.

Provides personal care to hospice patients at the VNA Hospice Center and in patient’s home.

We are a compassionate hospice organization dedicated to providing exceptional end-of-life care to patients and support to their families. We are seeking a dependable, caring, and patient-focused Home Health Aide (HHA) or Certified Nursing Assistant (CNA) to join our hospice team.


Position Summary

The HHA/CNA provides personal care and supportive services to hospice patients in their homes under the direction of a Registered Nurse and the interdisciplinary hospice team. The ideal candidate is compassionate, reliable, and committed to enhancing the quality of life for patients facing a life-limiting illness.

Responsibilities

  • Provide personal care including bathing, grooming, dressing, oral hygiene, and toileting assistance
  • Observe and report changes in patient condition to the RN Case Manager
  • Maintain accurate documentation of patient visits and care provided
  • Assist with light housekeeping related to patient care needs
  • Support patients and families with compassion, dignity, and respect
  • Follow the established hospice plan of care
  • Participate in a rotating on-call schedule to assist with patient needs after hours, weekends, and holidays as assigned


Requirements

QUALIFIED CANDIDATE WILL POSSESS:

  • Prefer minimum of 6 months as a CNA or HHA.
  • Current CNAs will receive training and certification as a HHA upon hire.
  • Must have certification in the State of Indiana
  • Must have and maintain a valid driver's license, and automobile insurance.
  • Must maintain current CPR certification.
  • Must be computer literate.
