Licensed Practical Nurse (LPN) – Skilled Nursing

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing team. 12 hour shifts 7a-7p one day a week, Wednesday and every other weekend.  

Key Responsibilities:

- Administer prescribed medications and treatments accurately and safely

- Monitor residents’ health status and report any changes to the nursing team

- Assist with activities of daily living, including bathing, dressing, and mobility

- Maintain detailed and accurate patient records in compliance with healthcare regulations

- Collaborate with registered nurses, physicians, and other healthcare professionals to develop and implement care plans

- Ensure a clean, safe, and comfortable environment for residents

- Follow all policies and procedures related to patient care and safety

Skills and Qualifications:

- Valid and current LPN license in the state of employment

- Proven experience in a skilled nursing or long-term care setting preferred

- Strong clinical skills and knowledge of nursing practices

- Excellent communication and interpersonal skills

- Ability to work effectively as part of a multidisciplinary team

- Compassionate, patient-focused, and attentive to residents’ needs

- Ability to handle sensitive situations with professionalism and discretion
