Position Overview:

Children’s Home Society of South Dakota is seeking a Transition & Training Coordinator to serve as a liaison between CHS therapeutic educational programs and public school partners.  As a leader in compassionate, trauma-informed care, Children's Home Society is committed to fostering equity and inclusion in all aspects of our work.

Compensation:

Starting Pay: $45,760.00-$57,200.00 (Depending on Education/Experience)



Schedule:

Monday-Friday 8:00 AM-4:00 PM

What You’ll Do:


  Day School Referral, Intake, and Placement Coordination

  • Coordinate the referral, screening, intake, admission, and placement processes for Therapeutic Day School students.
  • Serve as the primary point of contact for referring school districts and families, providing timely communication regarding referral status, requirements, and next steps.
  • Gather and review required referral, educational, and supporting documentation to facilitate screening and admission decisions.
  • Coordinate and facilitate student observations, program tours, intake meetings, and other pre-admission activities as appropriate.
  • Collaborate with appropriate CHS educational and clinical leadership to review referrals and support admission and placement decisions.
  • Coordinate information, communication, and timelines to support timely placement decisions and a smooth, successful student entry into programming.

Student Transition and Reintegration

  • Coordinate transition planning and implementation for students exiting Children’s Home Society School programs
  • Collaborate with students, families, CHS educational teams, referring districts, and receiving schools throughout the transition process.
  • Develop individualized transition plans addressing each student’s educational, behavioral, social-emotional, and support needs.
  • Coordinate communication, information-sharing, transition meetings, school visits, and other activities to promote continuity of services and supports.
  • Identify potential barriers to successful reintegration and collaborate with teams to develop proactive strategies that support transition to less restrictive educational settings.
  • Provide follow-up consultation and support to students, families, and receiving schools to promote adjustment, problem solve concerns, and strengthen transition outcomes.

School District Partnerships

  • Develop and maintain collaborative relationships with public school districts and other educational partners.
  • Serve as a liaison between CHS therapeutic educational programs and partner schools, promoting effective and consistent communication
  • Build awareness and understanding of CHS therapeutic educational programs, services, and referral pathways.
  • Identify emerging needs, trends, and challenges experienced by partner schools in supporting students with complex educational, behavioral, and social-emotional needs.
  • Collaborate with educational partners to problem-solve barriers, strengthen partnerships, and identify opportunities to better support student success.

Therapeutic Education Outreach & Training

  • Collaborate with appropriate CHS educational and clinical leadership to develop and provide training and educational resources for public schools and community educational partners.
  • Identify training needs and priorities in partnership with schools and districts and adapt content to their specific needs.
  • Provide training related to trauma-responsive and relationship-based practices, emotional regulation, behavioral support, classroom strategies, and sensory and environmental supports.
  • Translate CHS expertise in therapeutic education into practical, school-based strategies and resources that can be implemented within partner schools.
  • Coordinate training logistics, materials, communication, and follow-up resources to support effective delivery and implementation.
  • Gather participant and partner feedback to inform future training opportunities and continuously improve outreach efforts.

Educational Consultation

  • Collaborate with appropriate CHS educational leadership to provide consultation and practical support to partner schools serving students with significant behavioral, emotional, and educational needs.
  • Assist school teams in identifying  barriers to student success and developing practical strategies and supports within the student’s local educational environment.
  • Help teams translate therapeutic, behavioral, and relationship-based concepts into sustainable school-based practices.
  • Collaborate with school teams to identify and strengthen classroom, environmental, behavioral, and student-specific supports that promote successful participation in school.
  • Provide follow-up consultation and collaborative problem-solving as schools implement strategies and respond to changing student needs.
  • Support partner schools in building local capacity and sustainable practices that strengthen their ability to successfully serve students with complex needs.

Program Development, Data, & Quality Improvement

  • Support development, implementation, and continuous improvement of Therapeutic Day School and educational partnership initiatives.
  • Track and maintain data related to referrals, intakes, placements, transitions, outreach, training, consultation, and partnership activities.
  • Review data to identify trends, emerging needs, service gaps, and opportunities for program improvement or growth.
  • Gather and incorporate feedback from school districts, families, educational partners, and CHS teams to inform program development.
  • Collaborate with CHS leadership to evaluate program effectiveness, partnership outcomes, and progress toward established goals.
  • Contribute to quality improvement initiatives, reporting, and development of processes and resources that strengthen educational programming and partnerships.

Team Collaboration & Professional Responsibilities

  • Collaborate with CHS educational, clinical, residential, administrative, and leadership teams to support coordinated programming and effective educational partnerships.
  • Maintain effective communication and information-sharing across CHS teams to support students, families, and partner schools.
  • Participate in team meetings, committees, program development, and quality improvement activities as assigned.
  • Maintain required training, professional competencies, documentation, and organizational expectations  associated with the position.
  • Represent CHS professionally and collaboratively in interactions with students, families, school districts, community partners, and other stakeholders.
  • Demonstrate and promote CHS values, professional ethics, confidentiality, and collaborative problem-solving in all areas of responsibility.

Requirements

Education:  

Required

  • Associate degree in education, human services, behavioral sciences, social services, or a related field, or an equivalent combination of relevant education and experience.

Preferred

  • Bachelor’s degree in education, special education, human services, behavioral sciences, or a related field. Additional training or education in special education, therapeutic education, trauma-responsive practices, behavioral support, or related areas.


  

Experience: 

Required

  • Experience working with children and adolescents with educational, behavioral, and/or social-emotional needs. Demonstrated ability to build collaborative relationships and communicate effectively with families, professionals, and community partners.

Preferred

  • Experience in school-based or therapeutic educational settings, including collaboration with school districts. Experience with student transitions, referral/intake processes, consultation, training, or educational partnership development.


Licensure: 

Preferred

  • Current, valid South Dakota teaching certificate and/or special education endorsement
