As a full time Hospice RN Case Manager, you will coordinate and oversee individualized, patient-centered care for hospice patients in their homes, assisted living communities, and long-term care settings. As the primary clinical coordinator, you will manage all aspects of the patient's plan of care while building meaningful relationships with patients, families, and caregivers.

This role combines clinical expertise, compassionate care, care coordination, and patient advocacy. You will lead the interdisciplinary team in ensuring patients receive exceptional care that promotes comfort, dignity, and quality of life throughout their hospice journey.

Key Responsibilities

• Serve as the primary RN Case Manager responsible for coordinating and managing the patient's plan of care.  
• Complete comprehensive nursing assessments and ongoing evaluations of patient needs.  
• Develop, implement, and update individualized care plans based on patient goals and changing conditions.  
• Coordinate care among physicians, nurse practitioners, social workers, chaplains, hospice aides, pharmacists, and other team members.  
• Provide skilled nursing care in home and facility settings.  
• Manage pain and symptoms to maximize patient comfort and quality of life.  
• Provide patient and family education regarding disease progression, symptom management, medications, and caregiving strategies.  
• Advocate for patients and families to ensure care aligns with their goals, wishes, and advance directives.  
• Facilitate communication among patients, families, physicians, and care team members.  
• Respond proactively to changes in patient condition and coordinate appropriate interventions.  
• Participate in interdisciplinary team meetings and care conferences.  
• Maintain accurate, timely, and compliant clinical documentation within the electronic medical record.  
• Support quality improvement initiatives and regulatory compliance standards.  
• Practice independently while utilizing strong clinical judgment and critical thinking skills.

• A graduate of an approved school of nursing (RN/BSN/ARNP) with a current Iowa Nursing License. 
• Two years of home health or hospice or work related experience preferred. General medical knowledge related to home health and hospice services with end-of-life issues is desirable.
• Car, IA driver's license and adequate insurance as outlined in the personnel policies is required.  
• Strong clinical knowledge, critical thinking skills, and evidence-based practice expertise. 
• Excellent communication, interpersonal, and organizational skills. 
• Ability to prioritize tasks, delegate responsibilities, and manage multiple priorities effectively. 
• Proficient computer skills, including electronic health records (EHR) and Microsoft Office applications. 
• Advanced Certifications in Hospice and Palliative Nurse certification by the 3rd year of employment.
• All offers of employment are conditioned upon the result of a criminal background check conducted by the Iowa Division of Criminal Investigation, as required by law and a negative finding on the Office of Inspector General’s Excluded Provider List.  

Preferred

• Two years of hospice, home health, or related nursing experience    
• Experience providing care to individuals with serious or terminal illnesses    
• Knowledge of end-of-life care principles and symptom management
