The Family Partner works in the community support Walden Community Services program, a specialized agency within the statewide Child Behavioral Health Initiative. She/he is responsible for support and advocacy to parents/caregivers, providing some direct services and helping families identify strengths, needs and access to community resources through in-home visits. Spanish speaking skills preferred. One position will be in Springfield Part time and one position will be in Framingham Part time.
Walden Community Services Program Director & Senior Family Partner
FUNCTIONS AND RESPONSIBILITIES:
Works with families to identify a team to help families achieve goals.
Helps family identify strengths and needs.
Provides support services to family members such as accessing community support services.
Works with the Supervisor and team to develop a treatment plan with the family.
Responsible for maintaining paperwork required by the program.
Liaison with other providers as needed.
Provides formal and informal communication instruction/facilitation as needed.
Assists in the development of effective parenting skills as needed.
Utilizes a strength-based, family-centered approach to assist child and family in the development of self-advocacy skills.
Works well independently and as part of a team.
Demonstrates professional conduct and maintains confidentiality.
Maintains complete, accurate, and current records of all clinical services rendered.
Other responsibilities as determined by the Director of Walden School.
