**CASE MANAGER COORDINATOR**

**JOB SUMMARY:** 


Responsible for supervising Case Managers in the implementation and monitoring of Independent Support Plans in the day and residential programs. Provides leadership, guidance, and oversight to ensure high-quality service delivery, regulatory compliance, and staff development. Serves as the primary point of accountability for case management operations.  

Pay dependent upon experience.


**RELATIONSHIPS:** 


**REPORTS TO:** Program Director / Director of Services (or designee) 


**SUPERVISES:** Case Managers and related support staff 


**WORKS WITH:** Residential Coordinators, Registered Nurse, Program Coordinator, Day Services Coordinator, Day Program Staff, Residential Staff, Community Specialists, and other Supervisors. 


**EXTERNAL RELATIONSHIPS:** Independent Support Coordinators, Residents’ family members/guardians, Sister Agencies for Adults with Intellectual and Developmental Disabilities, State Department of Intellectual and Developmental Disabilities, Office of Community Services, State Quality Enhancement Team/Surveyors, East Tennessee Office of Licensure, City and County Fire Marshall, Department of Human Services/Adult Protective Services, physicians, and all others applicable.


  

**RESPONSIBILITIES:** 

1. Thoroughly learn, model, and enforce all Sertoma Center, Inc. Personnel Policies and Procedures in an unbiased and professional manner. Provide coaching and corrective guidance to supervised staff as needed. 

2. Thoroughly learn, enforce, and ensure staff compliance with all standards set forth by Licensure, Quality Enhancement, Medicaid Waiver, Fire Marshall, and the Center. Serve as the primary Officer for contract and regulatory compliance within the case management area. 

3. Supervise, train, mentor, and evaluate Case Managers to ensure consistent, high-quality implementation and monitoring of Independent Support Plans. 

4. Oversee caseload assignments, workload distribution, and performance standards to maintain service quality and regulatory compliance. 

5. Conduct regular individual and team supervision meetings; provide ongoing feedback, professional development opportunities, and performance evaluations. 

6. Review and approve documentation, reports, and plan updates prepared by Case Managers to ensure accuracy, completeness, and compliance with all applicable standards. 

7. Collaborate with interdisciplinary team members (Residential Coordinators, Nursing, Day Services, etc.) to coordinate services and resolve complex cases. 

8. Serve as a resource and escalation point for staff, families, guardians, and external partners regarding case management issues. 

9. Participate in quality improvement initiatives, audits, surveys, and corrective action planning related to case management services. 

10. Perform other related duties as assigned by leadership to support the mission and operations of Sertoma Center, Inc.

Requirements

  

**QUALIFICATIONS:** 


**EDUCATION:** 

Bachelor’s degree required (Master’s preferred) in Social Work, Sociology, Psychology, Special Education, or a related field. 

Alternatively, a combination of education and significant progressive experience working with individuals with intellectual and developmental disabilities may be considered. 


**WORK EXPERIENCE:** 

Minimum of three (3) years of experience working with persons with disabilities, including at least one (1) year in a supervisory, lead, or case management leadership capacity. 


**LICENSE OR REGISTRATION REQUIREMENTS:** 

Valid Tennessee driver’s license and a good driving record.

 

**PHYSICAL REQUIREMENTS:** 

Sight, hearing, and ability to perform occasional lifting or physical assistance as needed (e.g., supporting staff with client care situations).
