The Developmental Service Supported Independent Living Case Manager serves individuals diagnosed with an intellectual disability to achieve goals by facilitating the provision of services, coordinating with other service providers, and, most importantly, to develop a good working relationship with individuals on caseload.
CASE MANAGEMENT SERVICES:
Develops relationships and meets regularly with individuals on caseload; Schedules and facilitates collaborative meetings for ISA preparation; Facilitates and documents the person-centered program planning process (Needs Assessment, Person’s story, ISA); Collaborates with team and consumers to ensure that the ISA is implemented and is effective; Frequent alliance with team and guardian for updates and collective consultations. Ensures that all regulations are being followed; Performs medication administration, as assigned.
PROGRAM COORDINATION/PROGRAM DEVELOPMENT:
Ensures that all services are scheduled and being received by each individual; In conjunction with the ISA team members, develops individualized services for each individual; ensure goals, objectives, ISA, monthlies, and record keeping systems are developed accurately, written and in place in a timely fashion; Evaluate individual services for effectiveness, recommending changes to the QDDP when needed.
CLIENT ADVOCACY AND NETWORKING:
Respond to requests for information and/or technical assistance as appropriate; Refer individuals to appropriate services and resources; Maintain contact with other service providers (landlords, medical providers, employers, guardians, and other support services individuals are utilizing); Maintain contact with individual’s family members/support networks as appropriate; Serve as an advocate on behalf of individual, Developmental Services, and UCS. Function as a representative of the agency as assigned; Serve on an agency or division committees, as assigned.
Submit daily time sheets using correct codes for services provided. Write and submit case management notes, contact calendar, and other assigned documentation. Make sure there is all required documentation from other professionals.
