Salary Range: $17.50-$22.50 per hour plus benefits

Full/Part-time: Full Time 


Purpose of Position:

Responsible for engaging new members in admissions process for referrals for S.T.A.R. - Stand Together And Recover Centers, Inc. peer center and counseling services within 1 business day from date received. Conducts ongoing outreach and reporting of outreach attempts and members with no contact. Provides professional, warm, and inviting engagement during outreach to schedule intake appointments for new members. Collaborates with S.T.A.R. clinical team to make determinations and assess potential new members’ appropriateness for day program. Ensures accurate, complete, and quality enrollment paperwork is completed. Helps develop treatment plans that are individualized, recovery-focused, strength-based, inclusive of family and natural supports, culturally and linguistically appropriate, clinically sound, reflect the needs and goals of the individual, and results in improved health outcomes. Completes initial outcomes measurement tools during enrollment and works with team members to ensure additional tools are completed quarterly. Provides resources and follow-up for individual member needs (e.g. counseling, housing, etc.).

Essential Functions:

· Outreach and document successful and unsuccessful (contact v. no contact) attempts in the EHR (Credible) for new referrals within 24 hours

· Outreach and document ongoing attempts for members on “no attendance” reports 

· Consistently provides excellent customer service in all interactions with community and potential new members

· Educates potential members on what each S.T.A.R. service and/or program entails

· Schedules intake and orientation appointments for potential new members

· Provides friendly and clear explanation of S.T.A.R. enrollment paperwork and consistently completes it with members in its entirety

· Facilitates new member orientation delivered in both individual and group modalities

· Completes high quality enrollment paperwork and initial outcomes measurement tools (DLA-20 and ASU surveys), while developing rapport and engaging members to support their participation

· Serves as liaison for communication between Quality Assurance (QA) team and direct line staff at S.T.A.R. centers

· Schedules initial counseling sessions for new members approved for counseling 

· Works with the center receptionist and site management to guarantee outcomes measurements (DLA-20, ASU surveys, or other tools) are completed quarterly

· Establishes and maintains contact with relevant clinical team members from outside agencies during the admissions process

· Facilitates the development of a S.T.A.R. Individualized Service Plan (ISP)

· Participates in individual or group supervision in person or by a secure telephone or web-based connection with a Behavioral Health Professional (BHP)

· Facilitates two behavioral health prevention/promotion group a week

· Attend all staff meetings

· Other duties as required

Requirements

Education & Experience:
Must be able to be credentialed as a Behavioral Health Technician (BHT) within 90 days of hire.

AA/Bachelor/Master Degree (Preferred) 

High School/GED (Required) 

2 years of behavioral health experience (Required)

Other requirements:
·  Self-identify as a Peer by the International Association of Peer Supporters (INAPS)

or Mental Health America definition and/or have (or have had a history) of a
Mental Health/Behavioral Health or Substance Use/Abuse Diagnosis
·  Peer Certification within 90 days of hire 
·  At least 21 years of age
·  CPR/First Aid Certification 
·  Criminal Background Verification 
·  Pre-Hire and Random Drug/Alcohol Screening 

.  Covid-19 Vaccination
