This position is responsible for managing and effectively providing leadership to a 24-hour Transitional Care (TC). This position must develop professional relationships with TC staff and patients to positively facilitate the work of the team and promote optimum patient care. Works collaboratively with supportive living and marketing service lines to establish system delivery that supports the mission, vision, and values of VMRC.

Schedule:
- Typical schedule is from 8:00am-5:00pm, Monday-Friday, plus on-call rotation for weekends and holidays
- Flexibility with schedule is required, based upon the role's responsibilities
- Approximately 80 hours per pay period
- Eligible for benefits on date of hire
- Graduate of an Accredited School of Nursing with a current license to practice nursing as a RN or LPN in the Commonwealth of Virginia (BSN preferred).
- Past demonstrated supervisory, coaching, and support skills with personal care staff.
- Previous experience in long term care/SNF required.
- MDS, RAI, and care plan experience is preferred but not required.
- Must be able to effectively communicate orally, in writing, and through presentation using the English language.
- Maintain a professional working relationship with the Director of Nursing, employees, patients, physicians, families, and the public; ability to work collaboratively throughout the entire organization; effectively engage others.
- Knowledgeable and passionate about geriatrics, transitional, and long term care.
- Ability to supervise, coordinate, and effectively lead the activities of others.
- Highly organized, and willingness to be flexible to accommodate changing priorities.
- Self-motivated with proven ability to work both independently and in a team-based environment with minimal supervision.
- Looking for computer experience in Microsoft Office suite (Word, Excel, Power Point and Outlook) for operations, report generation, and presentations.
