Retention and Adherence Program (RAP) Specialist
Description

A. Provide clinical adherence management to clients in accordance with Policy, Procedure, and Regulatory Protocol. 

• Conduct initial assessment, intake, and service plan on each referral and develop a plan of care including medical, psychosocial, social, discharge, and interdisciplinary needs.

• Outreach/RAP Specialist will attempt to contact all potential and existing clients for the purpose of introducing, engaging, and re-engaging clients into VIP Health Services PrEP Program by phone, mail, and home visit and document all attempts.

• Evaluate clients bi-weekly and document progress.

• Reassess the client quarterly to re-evaluate current medical and psychosocial needs through systematic gathering of information to inform action steps of the RAP Specialist. This includes acknowledgment of success in addressing previously identified barriers to care and prioritizing the next steps to address any new barriers. 

• Participate in case reviews and interdisciplinary meetings to ensure coordinated delivery of services as scheduled or necessary.

• Coordinate discharge planning where appropriate or necessary.

• Provide ongoing education, support, and information to clients on pertinent medical issues including HIV.

• Facilitate support groups as assigned or indicated.

• Collect and supply data and information necessary for monthly reports to the Clinic Administrator or regulatory agencies.

• RAP Specialist will often be involved in scheduling and escorting clients to various appointments out in the community. 

• Produce and distribute marketing materials i.e., brochures, flyers, and posters. 

• Assist in planning, scheduling, and coordinating community events such as Community Health Fairs.

• Maintain a list of Outreach Partners and other pertinent contacts.

• Conduct Community Outreach activities through in-reach, outreach, and public education.

• Participates in community relations activities as directed by management.

• Represent VIP at Community Organizations and events.

• Establish and maintain a network of referral resources.

• Able to interact and advocate with other community service providers in a respectful and professional manner, identifying and building collaboration by scheduling community events/marketing activities to increase/engage in the RAP Program. 

• All other duties as assigned by the Clinic Administrator.


B. Maintain a Positive Image of the Organization in the Community the Organization Services and in State and National Organizations.

• Participate in Job Training and Education Programs, Meetings, and Committees.

• Attend mandatory in-service annually.

• Attend seminars to further professional development.

• RAP Specialist must participate in Team meetings, attend appropriate training, and follow all Health Services policies and procedures.




Requirements
  • Bachelor’s degree in human services and minimum one-year case management experience, or….
  • High School/General Equivalency Diploma and minimum 3 years case management experience.
  • Excellent written and oral communication skills.
  • Must have sensitivity to, interest in, and competence in cultural differences, HIV/AIDS, and minority communities. 
  • Ability to interact effectively with all populations served.
  • Ability to speak read and write the English language. Bilingual: Spanish speaker preferred. 
  • Completed NYS HIV Testing training preferred.

Salary Description
45,000 annually
