POSITION TITLE: Family Nurse Practitioner


POSITION REPORTS TO: Chief Clinical Officer


POSITION SUMMARY: Under the supervision and guidance of the Medical Director & Chief Clinical Officer, provides primary care services across the full age spectrum including health assessments, prescription of medications, and routine pediatric and gynecological care. Utilizes specialized knowledge and skills in Family Practice to ensure delivery of optimal care. Renders care to individuals in a manner that is consistent with current medical practices and is in compliance with all regulatory agencies.


POSITION FUNCTIONS:

· Performs complete history and physical examinations in accordance with current standards of professional practice, regulatory requirements, and organizational policies.

· Utilizes the nursing process, current standards of professional practice and standards of care in assessing patient needs and developing plans of care.

· Demonstrates competency in diagnosing and prescribing appropriate treatment interventions for the primary care patient and assessing the patient’s response to prescribed treatment modalities.

· Collaborates effectively with medical providers and other clinical disciplines in planning interventions that will best meet the patient needs in a timely manner.

· Prescribes appropriate laboratory / diagnostic procedures and consultative referrals related to the patient’s health status and facilitates appropriate treatment interventions predicated on results of these procedures and consultative reports.

· Identifies patient’s educational needs and utilizes appropriate resources to meet these needs.

· Consults with the Medical Director, Collaborating Physician, or Designee under the following circumstances:

o Patients who are acutely ill and may require hospitalization

o When deemed necessary by the FNP

o As requested by the Medical Director, the collaborating physician, or designee.

· Maintains open communication with all members of the health care team to promote a collaborative and collegial relationship within the health center.

· Collaborates and consults with the various programs within the agency and with outside agencies for additional health services and discharge planning.

· Demonstrates a commitment to maintaining an acceptable level of productivity by:

o Reporting to work on time and in accordance with assigned schedule;

o Facilitating the care of patients in a timely and efficient manner through appropriate triage of scheduled patients as well as walk-ins;

o Assisting other medical providers with heavier patient assignments and emergency situations;

o Demonstrating flexibility in scheduling to meet the needs of patient care and hours of operation

· Documents patient interventions and outcomes within the EHR in accordance with current standards of practice and organizational policies.

· Verbalizes and demonstrates knowledge and compliance with Adult/Pediatric Standards of Care, policies and procedures.

· Performs within scope of practice, demonstrates awareness of legal issues, and utilizes appropriate judgment and actions to reduce potential adverse consequences for patients, self and the organization.

· Participates in meetings, committee activities, peer review process, and the CQI programs, and mandatory trainings as assigned.

· Assesses personal learning needs and demonstrates a commitment to professional growth by attending continuing education seminars and maintaining current knowledge of health care trends, standards, and new technologies.

· Performs efficiently in emergency situations; remains calm, uses good judgment and follows established protocols.

· Provides a safe and secure environment for patients, family, staff and self through the appropriate application of established safety and infection control principles.

· Maintains adherence to Standard Precautions, OSHA standards for blood-borne pathogens, and infection control policies.

· Meets all organizational requirements at the time of employment and annually, thereafter.

· Verbalizes knowledge of and demonstrates sensitivity to the patient’s bill of rights, maintains patient confidentiality at all times, and adheres to HIPAA Privacy and Security Rules.

· Demonstrates initiative and competency in the utilization of Health Information Technology to perform essential job functions, which include but are not limited to (1) documentation in EHR, (2) transmission of Electronic Health Information, (3) retrieval of Electronic Health Information.

· Demonstrates respect for the cultural and religious diversities and needs of patients, families and other care providers.

· Creates and maintains an atmosphere of courtesy and respect towards patients, families and care providers.

· Demonstrates a commitment to building a supportive, team oriented work environment and projects a positive image to the communities we serve.

· All other duties as assigned by the Medical Director and/or Chief Clinical Officer.

MINIMUM QUALIFICATIONS:

· Graduate of accredited school of nursing, currently registered in New York State as a Registered Professional Nurse.

· Masters Degree in Family Practice with current certification as a Family Practice Nurse Practitioner and prescriptive privileges as outlined by New York State laws.

· One to three years of experience as a Nurse Practitioner in a primary or acute care setting.

· Clinical knowledge and experience in managing care of the substance using population is preferred.

· Ability to speak, read and write Spanish preferred.

· Excellent written and oral communication skills.
