Safe Parking LA - Case Manager

SUMMARY


SPLA Case Manager will be responsible for providing housing focused case management services to participants who are living in their vehicles enrolled in Safe Parking LA. Case Manager will provide direct program connection and engage in community outreach activities to ensure people experiencing homelessness living in their vehicles are familiar with Safe Parking LA. Responsible for client file management, reporting, ensuring outcomes are met and contract compliance.


ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:


- Initiate contact and establish a working interaction with participant for the purpose of providing assistance with attaining access to housing and services


- Provide case management services: develop and implement with participant, a comprehensive individual service plan based on assessed needs.


- Complete a CES (Coordinated Entry System) Survey to determine vulnerability and acuity of the individual experiencing homelessness, work to triage individual appropriate immediate needs (i.e. food, health care and crisis housing) .


- Provide housing support by obtaining housing readiness documents, assisting with housing search and coordinating housing appointments as needed


- Maintain thorough and accurate records in written form in case files and through the Homeless Management Information System of the Los Angeles Service Authority


- Maintain complete daily and mileage logs and monthly statistic reports and other program documentation in accordance with agency and funder requirements, oversee contract outcomes


- Participate in community meetings and represent the agency in outreach activities to support educating stakeholders about Safe Parking LA


- Other duties as assigned

Requirements

QUALIFICATIONS

- Bachelor’s degree or the equivalent combination of education and experience

- 2 years of prior work with people experiencing homelessness or at-risk people: Required

- Ability to work well with people of diverse backgrounds

- Ability to work independently with strong sense of focus, task oriented, non-judgmental, clear sense of boundaries

- Ability to look at situations from several points of view

- Good written and verbal communication skills, ability to articulate thoughts and ideas

- Excellent computer skills and proficient in Excel, Word, Outlook and Access A valid California driver’s license, auto insurance (as required by law), access to reliable and timely transportation to program sites and off-site meetings

- Background check (Live Scan) as necessary and required by funders or regulatory agency

- Spanish speaking highly desirable


Salary Range: $21.00 - $ 24.50  hourly - Depending on experience
