Requirements

The Admissions Registered Nurse (RN) makes patient referral visits and facilitates patient admission to hospice. Admission visits may occur at a patient's home, skilled nursing facility, residential care facility and hospitals. 


Job Responsibilities

  • Receive incoming patient referral related phone calls and coordinates follow-up as appropriate with physicians, families/caregivers and staff members
  • Develop a hospice presence in hospitals, SNFs, RCFEs with possibility of attending rounds and care conferences, conferring with providers, speaking at in-service meetings, attending committee meetings, and participating in new staff orientation
  • Act as a liaison and advocate for Hospice patients admitted to the hospital, SNFs, and RCFEs 
  • Establish professional working relationships with case managers, discharge planners and other referral sources at hospitals, SNFs, RCFEs
  • Assess each consult to determine the appropriated for Hospice admission, based on individual Hospice
    criteria 

Qualifications: 

  • Bachelor's Degree,  Associate Degree, or Diploma in Nursing from an accredited school of nursing 
  • Valid RN license in the State of California and current CPR certification
  • Minimum two years experience, at least one of which is in the area of acute care, or home care related setting. Hospice nursing is preferred
  • Self-directed, able to work with minimal supervision 
  • Excellent written and verbal communication skills 
  • Demonstrated ability to efficiently use a computer, prior electronic medical record (EMR) experience preferred 
  • Possession of a valid California driver's license, access to a reliable automobile, and minimum car insurance.
