  • Website Usage Data: As described in the Paylocity Cookie Notice

Sensitive Personal Information:

  • Race, gender identity, age, disability status, sexual orientation, veteran status
  • Government-issued identification numbers
  • Medical or background check information
  • Criminal history, as required for the position per CBRF and DHS licensing

FMG uses these communication methods to:

  • Deliver legally required notices

Consent: By applying to a role at FMG, applicants consent to receive such communications. Standard data/message rates may apply.

Opt-Out: Applicants may opt out of text messages at any time by replying “STOP” or contacting Recruiting@frontidacare.com.
