4. Employee Responsibilities

Employees must follow the Clean Desk Policy, lock away sensitive documents, use password-protected devices, log off unattended computers, and avoid unauthorized disclosure. Violations may lead to disciplinary action, including termination.

5. Access and Disclosure of Information

Access is granted only on a need-to-know basis. Unauthorized disclosure is prohibited. All company records must be returned upon termination.

6. IT and Data Protection Policies

- No recording of calls or meetings.

    - No use of unauthorized or AI-based applications.

    - Store data only in approved systems.

7. Monitoring and Enforcement

Periodic audits and inspections will be conducted. Personal items on company premises may be searched if a breach is suspected.

8. Rights to Access and Modify Personal Data

Employees may inspect their personnel files, update personal information, and request copies of certain records.

9. Data Retention and Destruction

Data is retained as long as necessary and destroyed securely when no longer needed.

10. Legal Compliance

This policy complies with the Ohio Data Protection Act, HIPAA, FCRA, GLBA, and local laws.

11. Changes to This Policy

Policy updates will be communicated in writing. Only the Chief Administrative Officer or Legal Counsel may authorize changes.
