Applicant Privacy Notice and Consent

Purpose
The purpose of this notice is to explain how the Practice collects, uses, maintains, and protects personal information provided by individuals who apply for employment.

Scope
This notice applies to all individuals who submit an application for employment with the Practice.


Collection of Applicant Information

As part of the recruitment and hiring process, the Practice collects and maintains personal information from applicants in compliance with applicable federal, state, and local employment and data protection laws.

If an applicant is hired, the information collected during the application process may become part of the individual’s employment record.

Applicants may request correction of incomplete or inaccurate personal information that they have submitted during the application process by contacting the Practice’s Human Resources department.

By submitting an application for employment, you acknowledge that:

  • You have read and understand this Applicant Privacy Notice. 
  • You consent to the collection, use, and processing of your personal information for recruitment and employment-related purposes as described above. 
  • You authorize the Practice to verify the information provided in your application and to conduct lawful background or reference checks as permitted by applicable law. 

Failure to provide required information may affect the Practice’s ability to evaluate your application for employment.
