The Migrant Seasonal Agriculture Worker Program Site Lead serves as the on-site migrant seasonal farmworker clinical leader and will serve as a resource to the clinical staff. The Site Lead will develop expertise in clinical aspects of delivering care to the farmworker population, especially in clinical areas that are unique to this population or especially prevalent in this population, such as pesticide exposure, occupational injuries, tuberculosis, and HIV/AIDS. The Site Lead will lead this program, including creating relationships with community organizations and individuals, bringing new patients into the practice, serving as a source of information about community resources for patients and staff, and promoting preventative healthcare, healthy living, and Westside Family Healthcare (WFH) services. This position develops and maintains relationships with farm owners, growers and other community agencies servicing migratory and seasonal agricultural workers and their families and oversees the mobile health outreach program.

  1. Manage the Migratory and Seasonal Agricultural Worker Program, including management of associated personnel and mobile health unit
  2. Recruit, interview, hire and evaluate the work of staff to ensure that programs are of appropriate quality and that resources are used effectively
  3. Ensure that program activities are conducted in accordance with grant requirements and with WFH policies
  4. Serve as the on-site migrant seasonal farmworker clinical leader, being informed, supportive of, and available to providers and staff
  5. Serve as the clinical content expert on topics related to farmworker health. Participate in CME activities, including farmworker conferences, to develop and maintain this expertise
  6. Actively participate in the Migrant Seasonal Farmworker Task Force
  7. Maintain the MHU in accordance with Joint Commission standards, including stocking and maintenance of diagnostic equipment, supplies and medications needed for patient care. Perform and document daily completion of QC logs and maintain POCT before beginning patient care
  8. Maintain excellent communication and collaboration with the site Office Manager
  9. Maintain an awareness of site clinical staff concerns; elicit feedback from site clinical personnel regarding concerns and ideas for suggestions
  10. Serve as a liaison between site personnel and WFH administration
  11. Serve as a liaison between on-site clinical and office staff
  12. Assist with/develop activities related to team development
  13. Establish and maintain relationships with other agencies and organizations in community to meet community needs and plan outreach events
  14. Identify community organizations or events where outreach activities can be conducted, and coordinate WFH’s participation in activities as appropriate
  1. State of Delaware (or compact state) Registered Nurse licensure with no restrictions or disciplinary action
  2. Minimum of 2 years’ experience as a Registered Nurse, preferably in the area of ambulatory care or comparable
  3. Current certification in Basic Life Support for Healthcare Providers
  4. Ambulatory Care Certification or commit to obtaining upon hire
  5. Five years of experience in the healthcare field, including experience working with vulnerable children and families
  6. Fluency in written and spoken English and spoken Spanish, including knowledge of medical terminology
  7. Current phlebotomy proficiency
  8. At least six months of experience with electronic medical record software
  9. Ability to travel independently to locations (reliable transportation, insured vehicle and valid driver’s license)
