Abilities First, Inc. is seeking a qualified professional to provide ongoing service coordination to families receiving Early Intervention services.
The Early Intervention Ongoing Service Coordination combines the traditional case management activities of organizing and coordinating needed services with the philosophy of family-centered care. A major goal of service coordination is to create opportunities for collaborative, family-centered, community-based services for infants and toddlers with disabilities and their families. Service coordinators help families identify and prioritize concerns, assist parents in developing Individualized Family Service Plans (IFSP)and strategies to meet the needs of their children and family units, and strengthen families' competencies and sense of control over life events. Service Coordinators help secure service providers to help meet goals identified and authorized in the IFSP.

The OSC will have the option of part time or full time work, with a flexible schedules that may involve weekends and early evening (depending on family availability)
Minimum of one of the following educational or service coordination experience credentials:
Demonstrated knowledge and understanding in the following areas:
Must have a current NYS driver’s license, free of moving violations; deemed acceptable by our insurance carrier.
Work hours and locations may vary and requires flexibility and time management skills
Involves travel, home visits and use of reliable transportation
