Description

The nature of the programmatic and modality work performed by this position includes Home Based Services, Adoption and Guardianship Support Services, Family Preservation Services, and Family Centered Treatment services.  This role will provide home visitation and therapeutic services for clients; document all contacts with clients; and, produce all required reports in a timely fashion.

Requirements

 • Provide structured goal-oriented, time limited therapy with families who need assistance in recovering from physical, sexual, and emotional abuse, and neglect. Other issues may be present and may need to be addressed in the course of treatment.
• Provide Adoption & Guardianship Support Services as needed and as assigned.
• Provide care according to evidence based practice standards.
• Provide ongoing support for clients, including emergency and crisis consultation on a 24-hour basis.
• Visit in client’s home no less than one time per week.
• Provide transportation for clients, as needed or necessary, to meet treatment goals.
• Engage in weekly staffing and address risk management areas to ensure safety of clients.
• Assist and teach through role modeling and hands-on education, guidance, parenting education and effective non-physical discipline/redirection of youth.
• Provide therapy and emphasis on identified areas of need. Examples include, but are not limited to: stress management, communication skills, conflict resolution, goal setting, behavior modification, family organization and structure, self-esteem, etc.
• Assist clients in finding and maintaining employment, if needed.
• Engage and assist the client’s participation in the treatment planning process.
• Accompany and support clients for court appearances.
• Demonstrate respect and cultural sensitivity to all clients and to all referral sources, and deliver services in a culturally        competent fashion
• Maintain strict confidentiality at all times to protect clients' privacy.
• Comply with required in-service / continuing education hours.
• Respect, support and integrate the company’s Tenets of Culture.
• Completion of clear, accurate, and timely clinical and agency documentation of 1.) all contacts with clients, 2.) monthly reports,      3.) court reports, and 4.) treatment plans.
• Attend all mandatory in-services / meetings.
• Attain / maintain CPR and First Aid certifications.
• Offer insights and input to strengthen the company’s performance and quality of services and to improve client satisfaction. 


Travel  

Travel includes driving typically within assigned district and will be determined by the needs of the clients.  An estimation of 200 – 500+ miles per week may be required. 


Physical and Visual Acuity Requirements 

The physical requirement of this position is defined as Medium Work: Lifting up to 50 pounds frequently. 

The visual acuity requirements needed to perform this position include having close visual acuity to perform activities such as: preparing and analyzing data and figures; viewing a computer terminal; and reading.

  • Presence of clients with emotional, behavioral, and/or mental health needs and/or challenges 

 

Required Education and Experience 

  • Master’s degree in social work, psychology, counseling, or marriage and family therapy 
  • Belief in the family preservation philosophy 
  • Knowledge of family of origin/inter-generational issues 
  • Knowledge of child and adult development and child abuse and neglect 
  • Knowledge of community resources 
  • 18+ months experience in child welfare and/or adoption/guardianship 
  • 21+ years of age 

Preferred Education and Experience 

2 years of experience in social work field
