As a case manager at Nashville CARES, you will develop relationships with clients and support them through many stages of life.
What You’ll Do
· Provide compassionate and respectful care and comprehensive case management services to help clients address the complex challenges associated with living with HIV
· Meet regularly with clients in the office, in clients’ homes, and in the community
· Conduct biopsychosocial assessments on all clients on a semiannual basis
· Assist clients with connections to medical, social, community, financial, and other needed services
· Assist clients in developing realistic action steps to achieve their goals
· Provide support services and referrals to clients who may be socially isolated
· Advocate for clients within the agency and in external settings, such as medical providers and other agencies they may be engaged with in the community
· Complete all documentation in an appropriate manner and in a reasonable timeframe according to agency standards to ensure clients do not encounter barriers to service
· Practice appropriate boundaries with clients and with work to ensure you are able to provide excellent services to clients while preserving self-care

Minimum Requirements:
· Bachelor’s degree (preference for Bachelor’s in Social Work or other Human Services)
· Minimum of two years’ experience in community based social or health care services – a supervised BSW/MSW internship may count for one year
· Proficiency with a variety of technology, including Microsoft Office software (Word, Excel, Outlook), electronic health records (training provided), and general Windows PC and cell phone technology
· Valid Driver’s License, vehicle, and automobile liability insurance to facilitate local travel required for client/agency-related work
