H2 Health is seeking a driven Occupational Therapist (OT) to work full-time between our Rehab Partners @ Crystal River and Rehab Partner @ Citrus Hills outpatient clinics! In this position, you will be working with a variety of populations and will be evaluating and treating patients with physical, cognitive, and/or developmental disabilities to help them develop, recover, and/or maintain skills necessary for everyday activities. You will work closely with other healthcare professionals to determine the best plan of care for each patient and use evidence-based methods and principles to provide effective intervention. You will also be responsible for documenting patient progress and discussing outcomes with other members of the healthcare team. Education: Graduate of an accredited Occupational Therapy program
Experience: Prior experience in Occupational Therapy is preferred; however, newly licensed occupational therapists will be considered.
Qualifications:
- Valid state license to practice Occupational Therapy
- Ability to assess a patient's level of functioning in order to develop a tailored treatment plan
- Excellent communication and interpersonal skills
- Ability to manage and prioritize multiple tasks
- Ability to work with individuals from diverse backgrounds
- Knowledge of the rehabilitation process
- Knowledge of community resources available to patients
- Ability to document patient progress and adjust treatment plans as needed
- Ability to work in an interdisciplinary team setting
- Ability to design and implement therapeutic interventions
- Ability to provide patient/family education
- Must pass required background checks
- Current CPR certification

Facility Hours: Monday - Friday from 8:00AM - 5:00PM
Hour per week: 40
Average Caseload: 10-12 patients/day
Compensation: Salary or Hourly, Commensurate with experience
