Description

CHC is looking for passionate team members who care deeply about the city of Detroit, the people they serve and the team they serve with. Community Health Workers are the engines that make CHC run and fuel our residents’ success! We are recruiting for multiple open CHW positions.


The Community Health Corps (CHC) compassionately advocates for residents of Detroit who need us most by providing tools to navigate barriers, accomplish goals, and move towards self-sufficiency. CHC addresses poverty and social disconnect through a restorative case management model, working alongside Detroiters for 3-6 months while they work towards self-sufficiency. 


CHWs connect vulnerable residents to resources and services such as food, transportation, rental assistance, and behavioral health services. Under direct supervision of a Case Manager, the CHW works on a team that conducts door-to-door outreach to residents in all Detroit neighborhoods. If not already certified, this position will be required to complete training for the role to become a certified Community Health Worker.

Requirements

Core duties and responsibilities for Community Health Workers:

1. Conduct initial home visit and ongoing outreach to residents in all Detroit neighborhoods.

2. Assess the needs of residents and collaborate with Case Managers to create a plan with goals, action steps, and timelines

3. Provide on-site resources or materials that ensure the immediate safety and health of participants and their families (e.g., food, child, or home resources).

4. Follow-up with residents and their families and connect them to resources as indicated by Case Manager.

5. Collaborate with program and operations team members for ensure processes are supporting client success.

6. Maintain accurate and timely case management notes and client records 

7. Facilitate outreach and resource attainment for a caseload of residents

8. Act as a liaison to City of Detroit departments and/or direct service provider community partners 

9. Drive a team of fellow CHWs to resident homes to support in the progression of their person-centered action plan
 

Desired Qualifications:

· Knowledge of community resources in Detroit

· Ability to work with people in crisis and conflict, including those facing homelessness, financial challenges, mental illness and/or chemical dependency

· Ability to develop good working relationships with internal and external stakeholders

· Ability to organize, prioritize multiple projects and meet deadlines in a time-sensitive environment

· Ability to communicate clearly and professionally verbally and in writing

· Ability to type and enter accurate data in program tracking and management systems

· Keen attention to detail, with the ability to identify inconsistencies in submitted documentation 

· Demonstrated ability to work effectively under stressful conditions

· Passion for working with and serving our neighbors most in need

· Maintain the confidentiality of clients as well as client information and records

· Working knowledge of Microsoft Word Office Suite (Word, Excel, PowerPoint)


Required Education and Experience:

1. High School Diploma or General Equivalency Degree (HSD/GED). 

2. Required to attend a mandatory orientation to the Community Health Corp (CHC), specific

training for the role and become certified Community Health Workers.

3. Experience with door-to-door outreach, customer service or other non-profit or community engagement work.


Licenses, Certifications, and Other Special Requirements:

· Valid State of Michigan Driver’s License – Position requires field work

· Community Health Worker Certification preferred


Physical Demands:

The employee generally engages in light physical activity with person to person contact inside and outside the office environment. The employee is expected to move about to accomplish tasks. Must be able to lift, push, pull, and carry up to 20 pounds.


Work Environment:

Work is performed in indoor and outdoor environments (e.g., home visits, office, care center, clinic. etc.). Work requires travel to various homes and businesses in all areas within the City of Detroit.
