While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

About the role:
An employee in this position provides direct casework and counseling services to the patient/family unit wherever they may be within the hospice program of care. The Social Worker will provide an assessment of psychosocial problems; define treatment goals and work to alleviate such problems. The Social Worker serves as part of the interdisciplinary team and shares responsibility for other aspects of patient care except where limited by licensure. Plans and coordinates patient care with the primary nurse under the direction of the Counseling Supervisor, Executive Director, and/or Regional Counseling Supervisor.
- Is a graduate of a Bachelor’s or Master’s program in Social Work accredited by the Council of Social Work Education; and is licensed by the state of Kentucky as a Licensed Social Worker (LSW), a Certified Social Worker (CSW), or a Licensed Clinical Social Worker (LCSW)
- A minimum of two years social service experience in a health care setting or social service agency. Experience working on an interdisciplinary team preferred.
- Knowledge of the process of death, dying and bereavement.
- Knowledge and basic understanding of the psychosocial and legal operatives in terminally ill patients and their families.
- Knowledge and basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary healthcare team.
- Current Kentucky driver's license and unlimited access to a reliable automobile.
- Ability to exercise judgment and discretion in confidential matters.
- Must be able to make home visits to patients/families in a variety of settings; mobile homes, apartments, homes, nursing facilities and condominiums that may not have elevators.
- Must be able to drive for extended periods of time with no ill effects.
- Must be flexible, good written and oral communication skills. Must have the ability to give and receive constructive feedback.
