Stavros is seeking to hire Contracted Nurses (RN’s/LPN’s) who would be meeting People with Disabilities (aka Consumers) in their homes, located in the Hampden, Hampshire and Franklin counties to complete initial and annual In-Home Evaluations and Re-Evaluations. These evaluations are essential in identifying the available resources and supports needed by the consumers.

We accomplish this through programs and services designed to meet the needs of persons of any age or disability as they work to achieve the life goals that are important to them.

Education, Experiences, and Skills:

• Must be able and willing to travel to complete in-home visitations and evaluations.
• Valid and unrestricted Registered Nurse / Licensed Practical Nurse license. 
• Current CPR certification. 
• Requires at least 1 year of community-based health experience.
• Knowledge of disabilities and community resources. 
• Proficiency with medical documentation and timely submission of documents
• Reliable transportation and proof of auto Insurance is required
• Bilingual preferred but not necessary (English/Spanish).
