Iowa City Hospice is a community based, non-profit hospice dedicated to providing compassionate care for anyone in our community affected by serious advances illness and end of life conditions.
We are looking for a talented FT RN to join our dynamic interdisciplinary care team. The Palliative Care Nurse is a “Field Based” position that assesses patient and family needs and then coordinates the patient’s plan of care with an interdisciplinary team and community partners.

Essential Job Functions (including but not limited to):
Complete the initial patient assessment and ensure that orders for treatment and services are obtained to meet the immediate needs of the patient.
Provide care that is in accordance with accepted nursing standards of practice and Iowa City Hospice policies and procedures.
Ensure the patient’s medication list is accurate through reconciliation with the facility or home medication list.
Ensure nursing care and services provided are in accordance with the patient’s plan of care and are based on the patient’s identified needs for the palliation and management of symptoms.
Supervise care delegated to a Licensed Practical Nurse and hospice nurse’s aide.
Assume primary responsibility of patient care management.
Communicate with the primary physician regarding patient change in status and pain and symptom control.
Document all care provided on the patient’s clinical record in accordance with Iowa City Hospice’s documentation requirements.
Attend and participate in patient care conferences to coordinate care with other healthcare agencies/providers.
Demonstrate exceptional written, electronic and verbal communication standards.
Establish and maintain strong relationships with Iowa City Hospice’s Care Partners.
Train patients, care partners and caregiver about symptom management , the safe administration of drugs and care giving techniques.
Coordinate ongoing patient care with the interdisciplinary team and Care Partners.
Ensure effective use of resources.
Serve as an educational source to staff, Care Partners and the Community.
Ensure implementation of strategic goals.
Adhere to the practice of confidentiality regarding patients, families, staff and the organization.
Participate in Iowa City Hospice’s success by ensuring access to our exceptional care.
Support and promote Iowa City Hospice’s mission.
Participate in the organization’s quality assessment and performance improvement program.
Perform other duties as assigned.
Iowa City Hospice requires clinical staff employed at 50% FTE or greater to obtain an applicable certification in hospice and palliative care, by the 3rd year of employment.
Licensure: Current Iowa license as a Registered Nurse in the State of Iowa.
Education: Nursing degree
Experience:Home Health, hospital and/or long term care experience preferred.
Driving: Proof of license and current insurance coverage.
