Reporting to the Program Manager, the Patient Coordinator will support the implementation of a national registry and database of cleft lip and/or cleft palate patients throughout Philippines. This position works closely with various community leaders and partners in the different states of the country to set up registration campaigns and camps.  This role will ensure all cleft patients in the region are located for surgery, as well as ensure any related follow-up care is provided to them.

Primary Responsibilities: 

• Set up meetings at hospitals and clinics across the regions, establishing key contacts at each health care facility. 
• Access existing community health care worker structures and arrange to implement cleft awareness programs within the community. 
• Set up meetings with traditional authorities in the areas. 
• Manage country-wide community mobilization through the creation and maintenance of a patient database.
• Meet with NGOs and community-based organizations within the Philippines to partner with them for patient recruitment or sponsorship.
• Manage the patient registration process; collecting patient data such as full names, addresses, and phone numbers. Provide assistance with coordinating patient travel to the program site or center.
• Assist program coordinator with putting together patient logistics (food, shelter, water, transport, reimbursements).
• Build strong relationships with patients and assess if their needs are being met. Seek assistance from program coordinators should patients and their families need support. 
• Create patient return lists and ensure all patients have transportation to return.

• Bachelor’s degree in related field 
• Licensed Social Worker 
• Prior experience building and maintaining databases 
• Fluency in English
• Experience in community health care mobilization 
• Excellent computer skills (Microsoft Office required) 
• NGO experience a plus
• Self-directed, strategic thinker with proactive problem solving skills
• Ability to travel frequently around the country
• Be self-motivated, able to plan ahead, and remain flexible, adaptable under pressure and with short deadlines.
