SUMMARY OF POSITION The Case Manager helps tenants of 2700 Wewatta identify and achieve goals that help them maintain safe, stable housing, employment, and education. This person guides tenants toward empowerment, community connectedness, self-sufficiency, and independence. The role may also include managing the front desk part-time. This will entail ensuring secure access to the building, meeting and building rapport with tenants, and assessing and intervening in crisis or safety incidents.


ABOUT 2700 WEWATTA: 2700 Wewatta is a 56-unit supportive housing complex for young people between the ages of 18-24 in which TGTHR is the contracted service provider. While living here, residents are offered employment and education assistance, support and inclusivity groups, counseling services, life-skills development, mental and physical wellness support, housing navigation when they leave, and an array of individual and group activities.


PRIMARY RESPONSIBILITIES: 

  • Provide support to tenants based on issues identified in the OneHome/HMIS Assessment, TGTHR’s Psychosocial assessment, and periodic life skills assessments.. 
  • Maintain a caseload of up to 28 tenants.
  • Monitor progress with voluntary participants on a monthly and quarterly basis, tracking tenant progress toward self-sufficiency via surveys, assessments, and other data collection methods. 
  • Be available for meetings as needed and as often as requested. 
  • Commitment to building a culture of mutual accountability, liberation, and respect. Adherence to TGTHR's core values of justice, equity, diversity and inclusion. 
  • Assist tenants with basic needs (laundry, shopping etc), personal finance, pro-social skills and life skills necessary to maintain housing. 
  • Assist tenants in developing a housing transition plan out of 2700 Wewatta and into self-sufficient housing. Advocate to leasing agencies for tenants receiving Housing Vouchers or other forms of assistance.
  • Complete all documentation using the appropriate date entry point including Salesforce, HMIS, and paper files.
  • Other duties as assigned by the Housing Programs Manager. 
  • Building trusting relationships with tenants and creating an environment that fosters independent living and community building.
  • Assessing tenants' needs, developing and documenting case plans, making appropriate referrals, and ensuring follow-through.
  • Maintaining professional boundaries.
Requirements

REQUIREMENTS & PROFESSIONAL QUALIFICATIONS

  • Bachelor’s degree in social services or a related field, 2-4 years of direct experience, or an equivalent combination of education and experience is required.
  • 3-5 years of experience working with vulnerable, at-risk and/or homeless young people in shelter, transitional or housing programs. 
  • Be able to manage time effectively.
  • Commitment to continued education of social justice, especially the intersection of youth homelessness and race, sexuality, gender, ability, and socioeconomics. 
  • Strong verbal communication, written communication, and interpersonal skills.
  • Able to understand and respect the importance of confidentiality and professionalism
  • Willingness and ability to work in a dynamic environment, in a team setting.
  • Strong computer skills, including GSuite and Microsoft Office. Salesforce preferred.
  • Valid driver’s license and reliable transportation
  • Ability to sit for long periods of time while also performing typing duties
  • Ability to lift a minimum of 30 lbs
  • Ability to lift, bend, stoop, kneel, crouch, push and other strenuous activities. 
  • TGTHR's important work classifies our employees and program participants as high-risk. Our staff's health, safety and ability to provide critical services remain our priorities. If you are hired, you will be required to prove that you have received the COVID-19 vaccine or are able to receive one within seven weeks of hire.
  • Knowledge of HUD and City of Boulder housing regulations preferred.
  • Knowledge of community resources in the Boulder area. 
  • Experience with transitional programs for homeless youth and young adults preferred.
  • Case Management experience preferred.
  • Bilingual, Spanish preferred.

Salary Range: $55,000-$60,000 per year

TGTHR is pleased to provide professional growth opportunities for full-time employees pursuing their LPC or MFT licensure. In exchange for providing counseling services in programs, employees will receive on-site individual and group supervision at no cost to the employee.
