Home Health Clinical Manager

Location: Cynthiana, KY (serves Harrison, Bourbon, Scott, and Nicholas counties) 

Schedule: Monday-Friday 8am-5pm; occasional on-call after hours and weekends

About the role: 

An employee in this position makes observations of the delivery and quality of client care and leads quality improvement activities;  reviews schedules for assigned worksites;  prepares work schedules and makes daily nursing assignments including any follow-up activities;  provides appropriate feedback to assigned staff including providing immediate, intermediate and formal summary performance evaluations;  participates in the disciplinary process of assigned staff as needed;  participates in meeting of supervisors and other administrative staff and models effective clinical decision making skills.  

Requirements
  • Is a graduate of an accredited school of professional (registered) nursing.
  • Is currently licensed as RN through the State Board of Nursing.
  • At least two year's general nursing experience is required with current hospice, home health, and medical, surgical or critical care experience.  Has experience with providing good pain management, IV skills preferred.
  • Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
  • Complies with accepted professional standards and principles.
  • Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
  • Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.
  • Possesses and maintains current CPR certification.
  • Have satisfactory references from nursing school, previous (or current) employers and/or professional peers.
  • Have excellent observation skills, good nursing judgment and communication skills.  Ability to provide good written documentation in a timely manner.
  • Is self-directed with the ability to work with little supervision.
  • Is flexible and cooperative in fulfilling role obligation and has the ability to work within an interdisciplinary team.
