DAMIAN FAMILY CARE CENTERS, INC. is a Federally Qualified Health Center with 14 community based health centers located in the metro NYC area, Suffolk County and Upstate, NY, providing primary care, behavioral  health  and specialized care at all sites. 


Position Title: Licensed Practical Nurse Reports To: Practice Administrator or Medical provider on duty 


Position Summary: • Provides direct patient care under the supervision of a Registered Nurse or Medical Provider and within the LPN scope of practice. • Supports the mission and philosophy of Damian Family Care Centers, Inc.  

Essential Functions: • Demonstrates initiative, competency and enthusiasm for carrying out daily assignments; assisting and taking direction from other health care providers in the delivery of patient care and maintaining patient flow. • Accurately collects clinical data on patients and demonstrates competency in performing clinical interventions. • Promptly reports identified abnormalities in clinical data to the Medical Provider or R.N. on duty. • Accurately documents nursing observations, interventions and patients’ response to treatments in the patient’s E.H.R.in accordance with established standards, policies and procedures. • Demonstrates a commitment to maintaining a supportive work environment that fosters collaboration and communication between all health care team members, projects a positive image to the communities we serve and demonstrates respect for the cultural and religious diversities and needs of patients, families and care providers. • Demonstrates the ability to identify and set priorities in the provision of care. • Demonstrates initiative and competency in the utilization of Health Information Technology to perform essential job functions, which include but are not limited to (1) Documentation in the Electronic Health 

Requirements

Qualifications: • Graduate of accredited school of Practical Nursing, currently registered in New York State as a Licensed Practical Nurse. • Minimum of one year’s experience in the ambulatory care setting preferred. • Skilled in phlebotomy • Current BLS certificate required • Current Infection Control Certificate required • Computer literacy with proficiency in Microsoft Word. Experience documenting in EMR • Ability to work independently and part of a team; prioritize tasks. Must have the ability to interface with participants/patients as well as all levels of staff. • Ability to interact effectively and professionally with persons from diverse cultural, socioeconomic, education, racial and ethnic backgrounds. • Must possess excellent organizational and communication skills. • Excellent verbal and written communication skills required.
