The Psychiatric Mental Health Nurse Practitioner (PMHNP) provides mental health services to patients with a wide range of behavioral health and substance use disorders and collaborates with the multidisciplinary teams to support the Behavioral Health Integration model of care at Westside Family Healthcare (Westside).

What you will do:

  1. Conduct mental health assessments, diagnosis relevant conditions, provide patient education, formulate treatment plans, and manage acute and chronic mental health needs, including substance abuse, within Westside’s Primary Care-Behavioral Health Integration model of care.
  2. Consult with medical providers on appropriate therapeutic drug regimens and other treatment strategies.
  3. Provide appropriate psychopharmacology interventions, including prescribing medications, ordering, and interpreting diagnostic studies and lab tests and other appropriate treatment in accordance with state and federal laws and regulations and in accordance with Westside policy.
  4. Coordinate appropriate referrals in collaboration with multidisciplinary teams.
  5. Support the stabilization of patients discharged from mental health hospitalization back into primary care for follow-up and ongoing management. 
  6. Use current evidence-based practice, research, and clinical knowledge to improve patient outcomes and reduce complications.
  7. Provide educational opportunities and training to Westside nurses, medical providers, and other staff, including education and precepting for students rotating at Westside. 
  8. Abide by all organizational, state, and federal policies and regulations related to confidentiality, and patient rights. 
  9. Maintain competence in clinical practice through continuing educational experiences and completion of required trainings to maintain licensure and certification.
  10. Participate in performance improvement and risk management activities, utilization review, and community education and outreach programs as requested. 
  11. Complete other duties as assigned.


Requirements
  1. Graduate of an accredited school or college of nursing with a Master’s Degree in Nursing 
  2. Current State of Delaware Registered Nurse licensure and Advanced Practice Registered Nurse Licensure (APRN) with a Certification in Mental Health and prescriptive authority, including controlled substance registration
  3. Board certified in psychiatric mental health (PMHNP-BC credential)
  4. Experience providing clinical consultations and trainings
  5. Basic life support for healthcare providers certification
  6. Ability to travel independently between worksites as needed
