Program Summary:

Casa De Rosemary and Orangetree are a housing continuum for transitional age youth (TAY) mothers and their children, providing up to four years of housing support.

Casa de Rosemary home is a transitional housing program that houses 5 mothers ages 18-25, along with their young children ages 0-5, in a shared home environment.

Orangetree apartments are the next step, allowing for TAY mothers and their children to pay a pro-rated rent and utilities in their own apartment.


Position Summary:

The Care Coordinator is responsible for providing direct, client led support and coordination of care for residents who are also parents. The Care Coordinator facilitates pre case management for potential residents to help transition them into the home, intake, and post case management to help youth transition to long term stable housing. This position requires a flexible schedule to accommodate the needs of the youth.

Requirements

Major Areas of Responsibility:

  • Provide direct support and care to residents using a trauma-informed, positive youth development approach as a core philosophy.
  • Coordinate weekly support sessions based on youth’s schedule and desire to participate, review progress on a regular basis to support the successful attainment of skills and goals.
  • Implement a positive youth development framework to support residents to maintain the home as a safe, youth-led, home environment, including resident facilitation of interviews with potential new residents and feedback to inform the programming and guidelines of the program.
  • Support residents to develop an awareness and understanding of individual and child safety using motivational interviewing techniques.
  • Provide crisis intervention and conflict resolution as needed, using therapeutic interventions that encourages and empowers residents to develop and use their own communication skills to discuss the issue causing the conflict.
  • Ensure the daily input of data into Apricot, and quarterly completion of Parent’s Assessment of Protective Factors (PAPF)
  • Review program applications, while assisting with decisions regarding a youth’s suitability for the program.
  • Coordinate the youth’s move in process, logistics of their move-in and ensure the emotional needs of youth and child are met.
  • Provide coordination and advocacy with outside service providers/volunteers on behalf of residents and attend case conferences and court as needed.
  • Maintain confidentiality related to program participation.
  • Schedule and co-facilitate/facilitate team meetings including Parent Coaches, outside providers, volunteers, interns or individuals who are part of the resident’s support system.
  • Perform other duties as assigned by supervisor.

Knowledge, Skill and Experience Requirements:

  • Bachelor’s Degree in a major relevant to the work or equivalent with two years of experience providing case management services to youth at-risk or individuals who are parents.
  • Knowledge of the issues faced by youth who are young parents, and have experienced a myriad of adverse childhood experiences placing them and their children at risk
  • Knowledge and understanding of working from a youth centered, trauma informed and harm reduction approach.
  • Excellent written and verbal communication skills, including experience and comfortability with public speaking.
  • Excellent computer skills including all Microsoft Office applications and the ability to effectively use database systems to perform the essential job functions
  • Pre-employment screenings include criminal background checks, FBI/DOJ fingerprinting, TB test, drug screen, and previous employment verification.
  • Must have a valid California driver’s license and proof of automobile insurance. Position requires DMV clearance

Working Environment/Physical Requirements:

Work is primarily performed in a two-story single-family home and multifamily apartment complex. Position will require use of personal vehicle. Physical requirements include standing, sitting, typing, bending and lifting up to approximately 50 lbs. (i.e. children, tables, chairs, food, water, etc.)

Salary Description
$27.00 to $35.00 per hour
