Home Health Clinical Manager
Location: Cynthiana, KY (serves Harrison, Bourbon, Scott, and Nicholas counties)
Schedule: Monday-Friday 8am-5pm; occasional on-call after hours and weekends

About the role:
A team member in this position makes observations of the delivery and quality of client care and leads quality improvement activities, reviews schedules for assigned worksites, prepares work schedules, and makes daily clinician assignments including any follow-up activities. The Home Health Clinical Manager will provide appropriate feedback to assigned team members including providing immediate, intermediate, and formal summary performance evaluations, and will participate in the disciplinary process of assigned team members as needed. The Home Health Clinical Manager will participate in meetings with supervisors and other administrative team members and will model effective clinical decision-making skills.
- Is a graduate of an accredited school of professional (registered) nursing.
- Is currently licensed as RN through the State Board of Nursing.
- At least two year's home health nursing experience is required.
- Supervisory experience 1-2 years preferred.
- Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles.
- Complies with accepted professional standards and principles.
- Possesses and maintains current CPR certification.
- Have satisfactory references from nursing school, previous (or current) employers and/or professional peers.
- Have excellent observation skills, good nursing judgment and communication skills. Ability to provide good written documentation in a timely manner.
- Is self-directed with the ability to work with little supervision.
- Is flexible and cooperative in fulfilling role obligation and can work within an interdisciplinary team.
- Must be a licensed driver with an automobile in good working order, that is insured in accordance with state and/or organization requirements, with personal auto liability insurance of $100,000/$300,000 in bodily injury coverage.
- Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
