Home Health Physical Therapist (PT)
Location: Cynthiana, KY
Schedule: Monday-Friday 8am-4:30pm, with occasional weekend on-call

About the role:
The Physical Therapist evaluates and treats patients upon referral by community referral sources in accordance with the order from the physician. Provides assessment and treatment in both clinical and residential settings through a variety of therapeutic procedures and activities, as well as the use of assistive devices, with the intent of maximizing functional potential and safety for the participant.
- Must be a graduate of a bachelor’s, master’s or doctorate program in Physical Therapy from an institution accredited by the Commission on Accreditation in Physical Therapy Education.
- Must be licensed in the state of Kentucky as a Physical Therapist.
- Possesses and maintains current CPR certification.
- Must be a licensed driver with automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- 3 years’ experience with geriatrics preferred.
- Additional experience with pediatrics is a plus, but not a requirement.
- Team members will be expected to comply with Bluegrass Care Navigators (BCN) Employee Health, infectious disease, and vaccination policies.
- Demonstrate integrity, consistency, & respect for differing views in interactions with others.

- Has knowledge of current professional practice standards, laws, & regulations.
- Has ability to develop, implement, & evaluate individualized plans of care.
- Understand role and responsibilities as member of the Interdisciplinary Team.
