Westside Family Healthcare is a nonprofit organization that provides high quality primary medical care without regard to ability to pay. The Cancer Screening Patient Navigator (RN/LPN) is a contracted position funded by the Delaware Division of Public Health. The Cancer Screening Patient Navigator (RN/LPN) will guide individuals through the cancer-screening process. This includes, but is not limited to, educating eligible individuals about cancer screening and associated processes, scheduling screening appointments, linking patients with cancer screening programs such as Screening for Life, and eliminating barriers to screening. The Cancer Screening Patient Navigator (RN/LPN) may also assess patient health problems and needs, maintain medical records and advise patients on health maintenance and disease prevention, and act as a clinical resource for the site Office Manager.

RESPONSIBILITIES

  1. Identify patients in need of cancer screening and guide them through the cancer screening process.
  2. Provide education and support regarding the need for cancer screening.
  3. Serve as an advocate for patients and providers by determining and meeting their needs as well as underscoring the importance of screening and being active participants in their own healthcare.
  4. Implement activities that are educational and serve to encourage individuals to receive cancer screenings.
  5. Keep patients’ personal health information confidential, discussing it only with other medical personnel who are involved in treating the patient or as otherwise authorized by the patient, required by law or permitted by Westside policy.
Requirements

MINIMUM OBJECTIVE QUALIFICATIONS

  1. Registered Nurse or a Licensed Practical Nurse with current Delaware licensure
  2. Associate’s Degree in nursing, health sciences or other health care related discipline
  3. Experience using computers, Microsoft Word, Excel, and Outlook and electronic medical records software
  4. Experience as a trainer or educator

PREFFERED OBJECTIVE QUALIFICATIONS

  1. Bachelor’s Degree in nursing, health sciences or other health care related discipline.
  2. 1-3 years of experience with health promotion, disease prevention, and health education in a community health setting.
  3. 1-3 years’ experience as a patient/ nurse navigator in a hospital, ambulatory care facility, or in a community/public health setting.
  4. Fluency in spoken Spanish strongly preferred.
