Position Summary: The LVN Clinical Coordinator plays a pivotal role in overseeing and optimizing patient care services. Working under the direction of a RN Clinical Supervisor, this role coordinates with and supports the clinical field staff. Responsibilities include; coordinating various aspects of care such as MD orders, patient triage, review and approval of supply orders, admission logs management, follow up and assigning labs as needed. They are integral in maintaining high standards of care and facilitating a smooth and supportive experience for patients and their families throughout their home healthcare journey.

  • Coordinate and oversee all clinical aspects of patient care, ensuring adherence to established standards and protocols. Appropriately escalate to Clinical Supervisor as needed. 
  • Actively involved in the daily operations and interacts with front line clinicians on a routine basis.
  • Collaborates with various parties; clinical leadership, operations, internal interdisciplinary team, patients, families, Physicians, and other stakeholders to ensure seamless coordination of care.
  • Ensures promotion and control of optimal patient care using outcome based and best-practice care planning
  • Serves as a resource and mentor to clinical staff, offering clinical guidance and support as needed.
  • Reviews patient documentation as it relates to the plan of care (POC) ensuring each patient is put on the appropriate clinical pathway ensuring positive patient outcomes
  • Responsible for Orders management; obtains verbal order from Primary Care Physician (PCP), drafts written order, submits to Clinical Supervisor for approval and sends to PCP for signature
  • Maintains logs including but not limited to Admissions, Discharge, and Infusion; tracks details to determine appropriate next steps (Discharge, Evaluation, Recertification or Resumption of Care) 
  • Reviews Home Health Resource Group (HHRG), Low Utilization Payment Adjustment (LUPA) rates in order to ensure proper utilization of services
  • Triages patient care events, initiates the proper protocol and collaborates with the Clinical Operations team accordingly. 
  • Participate in case conferences and interdisciplinary meetings to review patient progress and take the lead to address issues or concerns.
  • Actively assess and triages various communication channels; patient text threads, phone calls, Kmails, in order to ensure timely delivery of patient needs
  • Communicate effectively with patients and their families, addressing any questions or concerns related to clinical care.
  • Maintain open lines of communication with the clinical leadership team to report on clinical activities, outcomes, and challenges.
  • Responds to patient complaints, follows up as necessary with clinicians, escalates to leadership as appropriate and completes documentation of events.
  • Ensure that supplies are allocated appropriately to remove wastes and contain costs, processes requests in a timely manner to avoid delays
  • Reviews operational processes and presents recommendations in order to improve field clinician efficiency and satisfaction
  • Assist with regulatory surveys and audits, providing documentation and support as needed.
  • Facilitates Home Health Aide (HHA)s  on a monthly basis, including mandatory requirements (in the field and off the field). Identities and prepares important topics and organizes in - services for clinical staff,according to industry needs. Keeping record of in - services in yearly binders for easy access by surveyor, if needed. Electronic form of record keeping is possible after approval by management.
  • Stay informed about changes in regulations, industry standards and changes in policies and procedures
  • Supports leadership with ad hoc projects
  • Support operational functions such as; Authorizations and Medical Records

Requirements
  • Active and unrestricted CA LVN License 
  • Minimum of 2 years of clinical experience in home health or a related setting required
  • Strong clinical assessment and care planning skills.
  • Excellent communication and interpersonal abilities.
  • Demonstrate leadership capabilities and the ability to work effectively in a team environment.
  • Knowledge of Medicare regulations and OASIS documentation requirements.
  • Proficiency in electronic health record (EHR) systems and other applications.
Salary Description
$40-50/hr
