Description

The Program Manager of Rural Health Outreach manages the program providing mobile health outreach and education in Kent and Sussex Counties, including care for the Migratory and Seasonal Agricultural Worker (MSAW) population. This individual identifies outreach opportunities in Kent and Sussex Counties, including regular outreach to the farmworker population during the season, and develops and maintains relationships with community agencies, farm owners, and other individuals to facilitate outreach activities.


Core Responsibilities

  1. In collaboration with the Westside leadership team, support the organizational vision for rural outreach, maintain a Rural Health Outreach Program to achieve this vision.
  2. Oversee the Rural Outreach Program in Kent and Sussex Counties, including the day-to day direct supervision of community health workers and mobile health unit driver, maintaining team schedule, and performing quality audits. 
  3. Develop and maintain relationships with farm owners and key farm personnel (e.g., crew leaders) to facilitate Westside’s provision of care at local farms or to farm workers in the surrounding communities.
  4. Establish and maintain relationships with other agencies and organization which serve or support the farmworker population and general population living in Kent and Sussex Counties; identify opportunities for outreach activities and coordinate Westside’s participation in activities as appropriate. 
  5. Collaborate with the MSAW Clinical Lead to coordinate care for clinical services, and communicate all schedule changes. In collaboration with the clinical team, develop protocols and processes to optimize clinical care for the population served by the program, including addressing population-specific health issues such as heat-related conditions for the farmworker population.
  6. Provide leadership and support to, and participate on, the Migrant and Seasonal Agricultural Worker Advisory Council. Attend other community meetings events that support the vision of the Rural Outreach Program .
  7. Serve as a liaison with the communities served, educating them about Westside’s services. 
  8. Expand the reach of the rural health outreach program by supporting additional special populations, including Refugees, Tribal areas, and unhoused families. 
  9. Recruit, interview, hire, train, and evaluate the work of the program staff to ensure that the program provides high-quality care and optimizes access to care.
  10. Ensure that program activities are conducted in accordance with grant requirements and with WFH policies, including appropriate collection of required data. 
  11. Communicate with other Westside leadership to keep them informed about the Rural Health Outreach Program.
  12. Participate in leadership meetings as directed.
  13. Other duties as assigned.


Requirements

Minimum Objective Qualifications

  • Associates degree in relevant field or equivalent relevant experience
  • Five years of experience working in outreach programs
  • Three years experience working with grants and grant reporting
  • Ability to work nights and weekends.
  • Reliable transportation to travel between office sites, to venues served by the program and community events

Preferred Objective qualifications

  • Three years experience in supervisory/management role
  • Experience in Program Management
  • Fluency in spoken Spanish and/or French Creole
  • Experience working with the migrant and seasonal farmworker population
