Case Manager, Social Services
Description

The Case Manager is responsible for providing neighborhood based social and case management services to families and individuals with complex challenges.


Responsibilities (including but not limited to):

  • Provide quality case management to clients with complex needs.
  • Conduct needs assessment and determine eligibility for financial assistance.
  • Work with housing workers and clients to develop a budget plan and assist them with following through on budget goals.
  • Maintain a professional relationship with clients and conduct all business-related activity according to program policy and standards.
  • Maintain knowledge of current immigration laws and best practices and use knowledge to assist clients as needed.
  • Create and maintain client records.
Requirements
  • Bilingual in Haitian Creole or Spanish; highly preferred.
  • Bachelors degree; or equivalent experience.
  • Paralegal or legal training is desirable.
  • Experience and/or education in community development, direct social services, case management, or a related field.
  • Ability to work independently with minimal supervision.
  • Excellent written, verbal, and interpersonal communication skills.
  • Basic computer skills.
Salary Description
$22.00/hour
