Description
The Case Manager is responsible for providing neighborhood based social and case management services to families and individuals with complex challenges.
Responsibilities (including but not limited to):
- Provide quality case management to clients with complex needs.
- Conduct needs assessment and determine eligibility for financial assistance.
- Work with housing workers and clients to develop a budget plan and assist them with following through on budget goals.
- Maintain a professional relationship with clients and conduct all business-related activity according to program policy and standards.
- Maintain knowledge of current immigration laws and best practices and use knowledge to assist clients as needed.
- Create and maintain client records.
Requirements
- Bilingual in Haitian Creole or Spanish; highly preferred.
- Bachelors degree; or equivalent experience.
- Paralegal or legal training is desirable.
- Experience and/or education in community development, direct social services, case management, or a related field.
- Ability to work independently with minimal supervision.
- Excellent written, verbal, and interpersonal communication skills.
- Basic computer skills.
Salary Description
$22.00/hour
