Job Summary:
The Registered Nurse provides direct care in the skilled and long-term care setting. Communicates with physicians, Director of Nursing and coworkers, as appropriate, about changes in the patients’ clinical condition including results of diagnostic studies and symptomatology. Is able to respond quickly and accurately to changes in condition or response to treatment.
Responsibilities:
- Complete initial assessment on new patients within 8 hours of admission, including functional assessment, psychosocial, pain assessment, discharge planning, referrals to interdisciplinary team and code status
- Initiate and update patient care plan, assess patient educational needs and barriers to learning. Review all orders and transcribe when necessary.
- Knowledge of National Patient Safety Goals
- Accurately administers medications using the Rights of Medication Administration
- Ability to efficiently and accurately perform physical assessments on patients as indicated
- Provide intravenous (peripheral and central) site care as necessary
- Insert peripheral intravenous line when indicated
- Ensure crash cart and defibrillation function have been checked per policy and missing items/equipment are replaced immediately
- Comply with information in the Infection Manual and demonstrate appropriate use of PPE
- Ability to give detailed and efficient change of shift report
- Assists physician or practitioners with procedures on the unit as indicated
- Recognize signs of abuse and report to Social Services immediately
- Report abnormal test results or change in patient condition to physician in a timely manner
- Complete documentation in the EMR
- Provide education to patients and families as indicated and upon discharge
- Demonstrate thorough knowledge of the organization’s mission, vision and values
- Avoid and prevents injury to self and patients by applying principles of body mechanics
- Report all unusual incidents to the appropriate person and understand and abide by all departmental and hospital policy and procedures
- Attend scheduled and mandatory in-services
- Knowledge and adherence to ODH/CMS standards
- Comply with Code of Ethics and HIPAA requirements
- Knowledge of the hospital safety program and emergency codes and emergency preparedness procedures
- Maintain the department in a neat, clean and orderly manner eliminating any seen or known hazards
- Knowledge and compliance with standard and transmission-based precautions
Requirements
- Current and valid license as a Registered Nurse issued by the State of Ohio
- BLS upon hire and maintain current
- ACLS within 6 months of hire and maintain current
- Previous Acute Care hospital experience preferred
Required Skills/Abilities:
- Must be able to prioritize tasks
- Must be able to handle stress of working efficiently with frequent interruptions and distractions
- Must be highly motivated and willing to assume additional jobs and duties as needed
- Strong computer skills to include EMR and Microsoft Outlook

- Exposure to hazardous chemicals or infections
